Assorted deli meats and rare roast beef wrapped in such a way as to not protect from meat juices. Deli meats were stored in one container with rare roast beef, all stored in intermingling juices. Commercially processed, refrigerated, ready-to-eat, time/temperature-control-for-safety foods (roast beef, turkey breast, pastrami and sweet bologna) in the reach-in cooler and the walk-in cooler held more than 24 hours and are not being marked with the date they were opened; repeat violation, Oct. 21, 2022, and June 5, 2023. Paper towel dispenser empty at the hand-wash sink in the women’s room. Two large holes in the FRP behind the slicer. Rodent feces along the perimeter of the facility, on shelves, with plasticware and in the back storage area; repeat violation, June 5 and Sept. 27, 2023. Food facility inspection indicates evidence of rodent activity throughout the facility; facility does have a pest control program. Clutter, old equipment and debris in the back storage room.

Capicola, deli ham, salami, provolone cheese and sliced tomatoes were held at 51-52 F for an unknown amount of time in the bain-marie rather than 41 F or below as required. The ambient temperature of this unit was 61 F. Temperature-control-for-safety foods are not permitted to be stored in this unit until it maintains a temperature of 41 F or colder. Portable fans in back food preparation area had an accumulation of dust. Rags stored in the hand-wash sink in the front service area. Lights are not shielded or shatterproof over the sandwich preparation area in the back. Side door in the food preparation area is not self-closing and is being propped open. No sign or poster posted at the hand-wash sink in the front service area to remind employees to wash their hands.

Domino’s Pizza, 108 N. Reading Road, Suite K, Ephrata, April 22. Pass. Food employees in prep area not wearing proper hair restraints; a visor without a hair net is not acceptable. Food contact surface of plastic container covers are not all smooth, easily cleanable and/or resistant to pitting, cracking or scratching. Loose pieces dangling from edges. Toilet rooms do not have a self-closing door.

Hissho Sushi at Giant No. 6484, 1605 Lititz Pike, Lancaster, April 22. Pass. Food utensils in the sushi preparation area stored in a container of water that is not maintained at 135 F. Tuna vacuum-packed and received frozen, in the two-door cooler, has completely thawed and is still in original packaging despite package directions indicating contents should be kept frozen and removed from package to thaw under refrigeration.

The person in charge did not demonstrate adequate knowledge of food safety as evidenced by this noncompliant inspection. Nonfood contact surfaces (shelves and floors at kick plate) in the dried food aisles not cleaned at a frequency to preclude accumulation of dirt and soil. Potential rodent harborage areas in the dry warehouse storage area due to construction. Food facility is not inspecting incoming ingredients and supplies for the presence of pests, as evidenced by rodent dropping on dog food pallets.

The quaternary ammonia concentration in the sanitizing solution of the three-bay warewash sink was 0 ppm rather than 400 ppm as stated on the manufacturer’s use directions.
