The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Firehouse Subs, 2531 Market St. York, February 3. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed all spice shakers and bottles and large tubs/bins, ingredient storage containers, through-out the facility, is not labeled with the common name of the food. Observed three knives stored on a magnetic holder above the dirty dish side of the three bay sink area and subject to contamination through splash from rinsing dishes off. Observed a partial case of hoagie rolls and a case of small bagged potato chips stored directly on the floor in assembly area, rather than 6 inches off of the floor, as required. Observed eight cases of food on the floor in the walk-in freezer, rather than 6 inches off of the floor, as required. Four plastic containers of meatballs with red sauce, was only reheated to 75°F and 112°F for hot holding and not 165°F for 15 seconds as required. Employee was temping product with probe touching the container instead of inserted into a meatball causing the re-heating temperature to be inaccurate. Four plastic containers of meatballs with red sauce were held at 72°F and 112°F, in the hot hold warmer area, rather than 135°F or above as required. Observed assorted foods being held at 49°F, in the True three door baine marie in the assembly area, rather than 41°F or below as required. Thermometer in unit was inaccurate and reading 20°F. Pulled thermometer out of unit and was still not working. Temperature measuring device on the mechanical ware-washing machine in the back area was reading 110°F. After eight cycle runs of the machine, the probe inserted in the water finally read 120°F, while the measuring device still read 110°F and is in need of repair. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed three red deeply scored cutting boards not resurfaced or discarded as required at various areas of the assembly area in the front. Observed several large plastic tubs located on a shelf to the left of the three bay sink area, are not in good repair or condition (cracked, chipped and broken). Observed the following on food contact surfaces: Plug to slicer unit placed directly on clean catch area. Spoons, spatula, whisk, tongs with dried food residue, hanging on the wall over the single sink next to the dish-machine. Wall is not a clean surface, also causing contamination to these utensils. Peeler and corer hung over the three bay sink with dried food debris on utensils and not clean to sight or touch. Two plastic tubs on a shelf to the left of the three bay sink area with lids stored. Accumulation of dried crumbs coming in contact with the lids, bottles and shakers stored inside. Three containers on a shelf to the left of the three bay sink area with food debris inside of the containers coming in contact with the lids stored inside. Observed the mini mayor automatic toaster on the counter in the front assembly area with heavy accumulation of dried bread crumbs under the open conveyor portion of the unit, with the possibility to contaminated food being toasted. Discussed with Person in Charge who stated they do not take this unit apart to clean it. Observed heavy accumulation of dried food debris, crumbs, trash, and rags in the space between the walk-in cooler and walk-in freezer units. Observed an accumulation of heavy static dust on the wall above the dish-machine and three bay sink area, including the piping and wiring to dish-machine, with the possibility to contaminate clean dishes below. Observed ceiling tiles, light covers, and ceiling vents in the front and back of the food facility, with accumulation of static dust over food prep, cook and assembly areas with the possibility to contaminate food being prepared/stored/assembled below. Observed single-service, single-use articles stored in assembly area and on a shelf in the back prep area which were not in the original protective package or inverted to protect against contamination. Observed three cases (cups/lids) single-service, single-use articles stored in area directly on the floor at the entry to the back stock area, and not 6 inches above the floor. Observed a partial case of paper boats on the floor between the tables at the assembly line and not 6 inches above the floor as required.

Home Assn. Of VFW Post #2493, 195 S. Main St. Mount Wolf, February 1. Pass. Observed interior shelving and floor of beer cooler, in the bar area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Hood ventilations system baffles have an accumulation of oil and dust and are in need of cleaning.

Marcello’s Pizza, 421 Center St Mount Wolf, February 1. Pass. Raw shell eggs were stored over ready to eat foods in the bain marie. Corrected. Turbo air door interior surface is insulation, sheet metal panel is missing, foam insulation is porous and not cleanable. Observed smallware storage shelving, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces.

Starbucks Coffee Co #3481, 14605 Mount Airy Rd. Shrewsbury, February 1. Pass. Loose rubber door gaskets observed on the sandwich freezer in the back room. The handwash sink in the back room dishwashing area was blocked by a large trash can and not accessible at all times for employee use. Corrected.

Sweet Frog, 14635 Mount Airy Rd. Shrewsbury, February 1. Fail. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Fresh blackberries observed to be moldy and is adulterated. Corrected. Facility discarded food. Observed containers of yogurt mixes being stored on floor of refrigerator units that had clogged drains causing pooling of water which was subjecting yogurt containers to potential contamination. Refrigerated ready to eat, time/temperature control for safety food – cut melon, kiwi prepared in the food facility and held for more than 24 hours is not being date marked. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturers use directions. Observed inside walls and floor of refrigerator units holding containers of yogurt mixes with an accumulation of dirt, food residue, debris on non-food contact surfaces. Observed surfaces of carts being used to transport food with accumulation of old food residue and soil accumulation. Observed boxes of opened single use gloves, spoons, and cups stored in back room area directly on the floor, and not 6 inches above the floor. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Handwashing sink located in food service area was not operational at the time of this inspection. Water had been turned off due to a leak. Corrected. Facility turned on water. Old food residue, dishes and utensils observed in the handwash sink, indicating uses other than handwashing. Corrected. Soap was not available at the handwash sink in the food service area. Corrected.

Trail Pizza And Subs, 4265 N. Susquehanna Trail, York, February 1. Pass. 4X4 untreated wood brace observed between two units in the kitchen area is not smooth, non-absorbent and corrosion resistant as required. Observed single-service, single-use articles (napkins, drinking cups) stored in game room area directly on the floor, and not 6 inches above the floor. Lights are not shielded or shatter proof over the sandwich preparation area.

Corrected. Corrected. Freezer door is damaged and gasket is torn. Floor of freezer has several broken tile that are in need of repair.

Imperial Gourmet, 227 Pauline Dr. Suite 11, York, January 31. Pass. Several raw animal foods (chicken and shrimp) were stored above ready to eat foods in the walk in refrigeration unit. Corrected. Bowls were being used as dispensing utensils in the bulk dry foods. Corrected. Observed wet wiping cloths in kitchen area, not being stored in sanitizer solution. Corrected. Mama D’s Bakery & Cafe, 801 Delta Rd. Red Lion, January 31. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food Employee observed donning single use gloves without a prior hand wash. Food employees observed in kitchen prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Raw eggs were stored above cooked chicken breasts in the refrigerator unit. Corrected. Observed wet wiping cloths in food prep area, not being stored in sanitizer solution. Observed blueberry scones and an open can of apple juice stored uncovered and unprotected in a reach in freezer that had severe ice accumulation that was discolored and was subject to potential contamination. BBQ ham, which was cooled, was not reheated to 165°F within two hours, for hot holding. Corrected. Facility heated food to 165 degrees on stove top prior to placing into hot holding. BBQ Ham was held at 94 °F, in the service area, rather than 135°F or above as required. Corrected. Paper towel dispenser empty at the handwash sink in the kitchen prep area and also ladies bathroom. Corrected. Observed a pair of employee jeans thrown over door of food storage unit. Observed coat and phone and keys lying on shelf over single serve articles. Food facility does not provide lockers or storage for food employee personal clothing and possessions. Observed old standing dirty mop water in mop bucket. Mops are not being hung to air dry.

Olive Garden #137, 1340 Kenneth Rd. York, January 31. Pass. Fryer baskets with loose and broken wires, not smooth and easily cleanable, replace or repair. Outside waste handling unit and storage area was extremely dirty, sticky, and attracting insects and rodents. Please dispose of garbage in receptacles. Dumpster lid and doors were left open in not active use. Vents and Air returns in prep kitchen area, cook line and expo line with an accumulation of static dust and needs to be cleaned. Food facility is using Quat Sanitizer at an extremely high concentration of 500+ ppm, not approved in the Code of Federal Regulations for food contact sanitizing at this level. Facility of other means of sanitizing and will discontinue use at 3 compartment sink and use dispenser in mop sink area until corrected.

Oyako Sushi, 3619 E. Market St. York, January 31. Pass. Food utensils in rice steamer area observed stored in a container of water which is not maintained at 135°F. Corrected. Frozen eel thawing at room temperature in the sushi area. Fish was not removed from vacuum sealed package prior to thaw per manufacture instruction. Corrected – provided training. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods (sushi rice) without written procedures or documentation to verify disposition of food. Corrected – provided training. Table mounted can opener cutting blade, a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected. Observed three cooling units in the kitchen area that are not in use, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces.

Rocoto (Peruvian & Mexican Grill), 2997 Cape Horn Rd. Red Lion, January 31. Pass. 3 squeeze bottle food storage containers, in the salad making unit, are not labeled with the common name of the food. Corrected. Refrigerated ready to eat, time/temperature control for safety food – empanada filling, cooked beef, salsas prepared in the food facility and held for more than 24 hours, located in the prep refrigerator, is not being date marked. Corrected. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 00 ppm, rather than 200 ppm as stated on the manufacturers use directions. Corrected. Facility will manually mix and test sanitizer solution until dispensor is fixed. Mechanical warewashing equipment had a lime buildup observed inside.

Taco Bell #037076, 1517 N. George St. York, January 31. Pass. Condenser unit in walk in freezer observed with accumulation of ice on pipes and surrounding walls and shelves, not in good repair. Blue bucket of sanitized water at front counter observed reading 0 PPM rather than 200-400 PPM per manufacture requirements. CORRECTED. The food facility does not have the original certificate for the certified food employee posted in public view. CORRECTED. Static dust observed on fan guards in walk in cooler.

The Steakout, 2813 E. Prospect Rd. Suite 1, York, January 31. Pass. Observed wiping cloths in extremely unclean condition in warewashing area. Seafood soup, a refrigerated ready to eat time/temperature control for safety food, inside the upright refrigerator, was not compliant with date-marking by being labeled with a discard or use by date of no more than 7 days, and requires discarding. Operator verified soup was prepared and cooked yesterday. Corrected – Date marked per food code regulation. Condensate drip trays/lines in the soda can refrigeration case are not sloped to allow for proper drainage of condensation as evidenced by pooling water inside the unit. Manual warewashing equipment observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day. Observed fryer oil barrel in the warewashing area, with encrusted grease and soil accumulation. Observed fryers, in the kitchen area, with encrusted grease and soil accumulation. Cleaning equipment observed in the handwash sink, indicating uses other than handwashing. Corrected. Exit door located in the rear area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals.

House made cocktail and honey mustard sauces, refrigerated, ready to eat time temperature control for safety foods, were date-marked by the facility, but beyond the 7 day use or sell by date and requires discarding. Corrected – 2 lb discarded valued by Person in Charge at $4. The hot water used for sanitizing in the mechanical warewashing machine did not reach 180°F (or 165°F for stationary rack). House prepared pizza sauce per company policy has a 10 hour shelf life. In use sauce was dated/timed 1-29 7:41 p.m. Sauce was discarded weighed and valued by manager. 20 lb. $15.
