The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Fuddruckers 7332, 1221 W. Chocolate Ave. Hummelstown, December 16. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed two employee cell phones stored on plastic shelving over raw bagged carrots at the doorway near the three bay sink area, over ready-to-eat food. Three employee’s beverage containers were observed on the plastic shelving in the hallway near the three bay sink and above the vegetable prep table across from the three bay sink area, a food preparation and storage area. A female food employee observed on the cook line area, not wearing proper hair restraints, such as nets or hats. Observed several food ingredient storage bottles on the cook line area, is not labeled with the common name of the food. Food dispensing utensil in seasoned flour observed stored in the food and not with handle above the top of the food and the container. Also, scoop had dried accumulation of flour on it and had not been cleaned at a frequency to prevent contamination. Loose/deteriorated rubber door gaskets observed on the single door baine marie cooling unit on the cook line. Observed two green lids, at the cook line and the milk-shake area, are not in good repair or condition. Observed two mechanical ware-washing racks with accumulation of food debris on the interior and exterior of the racks contaminating the dished being cleaned in them. Observed numerous ladles hanging over the three bay sink area and are not stored at a height to prevent contamination from employee grabbing utensil at the food contact end of the ladle. Observed dried food debris on the underside motor housing of the right milk-shake blade that was dried from the night before, with the possibility to contaminate food being prepared below. Observed single-service, single-use articles stored on the cook line area and the plastic shelving near the three bay sink, which were not in the original protective package or inverted to protect against contamination. Observed three boxes of single-service, single-use articles stored in dry storage area directly on the floor, and not 6 inches above the floor. Observed walls in the back corridor area, has numerous hole, or is broken and in need of repair. Also, outlet plate is missing in this same corridor. Old unused equipment stored in dry storage area and the hallway dry storage area, should be removed from food facility.

Burger King #812, 4255 Union Deposit Rd. Harrisburg, December 14. Pass. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration. Loose water tray observed in the interior of the ice machine; in need of repair. The quaternary ammonia concentration in the sanitizing solution of the temporary sanitizing buckets in the kitchen area were 50 ppm, rather than 200-400 ppm as stated on the manufacturers use directions. Corrected. Carpet used in walk-in cooler area, is not closely woven and easily cleanable. Wall, in the office area, and is not durable, smooth, non-porous, non-absorbent; in need of repair. Exterior surfaces of the fryers, grease hoods, and ceiling vents throughout the facility area of the food facility is extremely dirty, dusty, and in need of cleaning.

Chautari Restaurant, 5890 Derry St. Harrisburg, December 14. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Raw chicken, pork and goat meats were all thawing in the same bowls in the three-bay manual ware-washing sink, with no barriers to prevent cross-contamination. Corrected and discussed with person in charge. Various foods including prepared noodles and cut vegetables stored open with no covering in the reach in refrigerators. Corrected and discussed with person in charge. Raw chicken, pork, and goat meats were observed thawing in standing water in the 3-bay manual ware-washing sink, which is not an approved thawing method. Corrected and discussed with person in charge. Raw shelled, washed eggs were stored at room temperature in the kitchen table area. Corrected by voluntary disposal. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the reach-in refrigerator, is not being date marked; i.e. cut vegetables, prepared noodles, etc. The handwash sink in the front kitchen area was blocked by a curtain and not accessible at all times for employee use. Corrected. Food employee personal food items observed stored among customer prepared foods in the reach in refrigerator. Corrected and discussed with person in charge. Grease hoods above the cook line area of the food facility are extremely dirty, dusty, and in need of cleaning.

CVS Pharmacy #2370, 3865 Derry St. Harrisburg, December 14. Pass. No violations.

Dollar General Store # 14049, 427 Second St. Highspire, December 14. Pass. Observed heavy accumulation of dust and spider web on the ceiling and wall in the back storage area. Two unisex toilet room is not provided with a covered waste receptacle for sanitary napkins. Exit door located in the back storage area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Wall in the back storage area, has numerous holes in need of repair.

Sportsman’s Golf Course, 3800 Linglestown Rd. Harrisburg, December 14. Pass. Food Facility is not using an approved detergent-sanitizer in both compartments of the two compartment manual warewashing equipment.

Villa Schiano, 4900 State Route 209, Elizabethville, December 14. Pass. Food Facility receiving shell eggs which are not labeled properly and/or do not contain safe handling instructions. Observed a pot of soup stored directly on the floor in walk-in cooler, rather than 6 inches off of the floor as required. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was

Papa Joe’s Pizza & Subs, 3950 Tecport Dr. Harrisburg, Type 2 Follow-up, December 13. Pass. No violations.

St. Thomas Roasters, 5951 Linglestown Rd. Harrisburg, December 13. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Countless containers with coffee beans stored in the front area, stored open with no covering. Observed food ingredient storage containers and bottles, throughout the facility, is not labeled with the common name of the food. Observed wiping cloths in bad conditions and deteriorated. Sponges observed in the prep-kitchen area being used to clean food contact surfaces. Observed deeply scored cutting boards not resurfaced or discarded as required. Observed the following on food contact surfaces: Ice machine with brownish mold-like substance on the interior dispensing bar and not clean to sight or touch. Facility has the Food Hander License Certification expired. The handwash sink in the prep-kitchen area was blocked by numerous food utensils and not accessible at all times for employee use.

The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Corrected. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the walk-in cooler, and held more than 24 hours, is not being marked with the date it was opened. (breakfast wraps and burritos). Breakfast wraps and burritos, a refrigerated, ready to eat time temperature control for safety food in the walk-in cooler area, was date-marked by the facility, but was beyond the 7 day use or sell by date and requires discarding. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the reach in refrigerator equipment. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturers use directions. Corrected. The handwash sink located in the ware-washing area does not have water at a temperature of at least 85°F. Women’s toilet room is not provided with a covered waste receptacle for sanitary napkins. Toilet rooms do not have a self-closing door. Paper towel dispenser empty at the handwash sink in the bathroom area. Toilet Room door was propped open, and not kept closed as required.

Subway @ Harrisburg International Airport #1 (inside), 1 Terminal Dr. Middletown, Follow-up, December 12. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the front under counter cooling unit. Observed numerous pieces of equipment in the back area that are deeply stained, cracked, broken and in need of replacement. Observed deeply scored cutting boards not resurfaced or discarded as required. Four cutting boards at the assembly area and one cutting board at the back slicing/prep area under the slicing unit. Observed the following on food contact surfaces and were not clean to sight or touch: Two knives at the assembly area of the facility are stored in containers with dried food debris and crumbs and are contaminating knives stored. One knife stored in a dirty metal container, with a paper towel in the bottom, in the back prep area contaminating knife stored inside. Wash, rinse and sanitize sinks at the three bay sink area with heavy accumulation of food and grease and not cleaned before use and contaminating cleaned utensils, dishes. Gloves and trash (paper towels) stored directly on the cutting board at the hot station contaminating cutting board for food preparation. Slicer with food crumbs and not thoroughly cleaned. Food debris, a pair of dirty hot mitts, ham, pita flat breads directly on the prep counter beside the warmer and counter cook oven. Observed numerous bins stored under the prep table in the back area, with utensils, lids, bottles, etc. stored inside, with food debris and crumbs on the food contact surfaces.

Perkins Family Restaurant, 7833 Linglestown Rd. PO Box 6655, Harrisburg, December 11. Pass. Loose rubber door gaskets observed on the two doors Delfield cooling unit. Observed the following, throughout the facility area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces: Exit door; All drains; Wall in the storage area of cleaning equipment; Hang sink in the kitchen area; Floor in the dry storage area. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil.

Ted’s Bar and Grill, 7300 Allentown Blvd. Harrisburg, Change of Owner, December 11. Pass. All food ingredient storage containers, in the food facility, is not labeled with the common name of the food. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Loose rubber door gaskets observed on the Avantco up right two doors and three doors cooling units. Accumulation of grease and dust on the range hood, which is dripping on the cooking area below and contaminating food being prepared.

Turkey Hill Minit Market #034, 170 E. Main St. Middletown, December 11. Pass. Observed dried, dripping and solidifying ooze coming out of the milk/cream, cappuccino, an flavor station dispenser nozzles. Discussed with Manager an corrected on sight. Observed exterior of four lid dispensers with dried food debris and not clean to sight or touch.

Wal-Mart Store #1591, 6535 Grayson Rd. Harrisburg, December 11. Pass. Observed deeply scored cutting boards not resurfaced or discarded as required, in the meat department. Paper towel dispenser empty at the handwash sink in the bakery area. Corrected. The following areas of the food facility are extremely dirty, dusty, and in need of cleaning: ceiling vents throughout the back bakery area; meat department walk-in cooler doors.
