The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Markethouse Coffee Corner, 15 S. Third, St., Columbia, Dec. 23. Pass. Food employee preparing food wearing a wristwatch. Prepackaged muffins and cookies are not labeled properly with the name of product, ingredient statement and distributed-by statement. Prepackaged muffins and cookies are not labeled to clearly indicate any “Big 9” allergen ingredients and/or the allergen warning statement. Food facility does not employ a certified food employee as required. The facility has 90 days to enroll an employee in a state-recognized food safety course for managers. Food facility has an employee that has taken food safety training program; however, the food safety program was not an ANSI/CFP-accredited certified food manager program. Food facility has a reminder statement on the menu for foods that are/or could be served raw or undercooked to the consumer; however, does not clearly disclose to which foods the reminder statement applies. Brownish biofilm residue on inside of ice maker deflector plate. Static dust on the air return vents above food prep. No “employees must wash hands before returning to work” signage in men’s and women’s public restrooms. Hot line grill prep sink leaking at the spigot. Ceiling tiles missing in the chemical storage room and need to be replaced.

Hokkaido Sushi & Hibachi, 1623 S. Market St., Elizabethtown, Dec. 21. Fail. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Multiple food items in the walk-in cooler stored open with no covering. Raw chicken stored on top of raw beef in common equipment with no barriers to prevent cross-contamination. Bulk food ingredient storage containers (rice, flour) in the main kitchen prep area are not labeled with the common name of the food. Food-dispensing utensil in bulk flour and rice bins stored in the food and not with a handle above the top of the food as required; corrected. Food facility is reusing No. 10 cans for storing cleaners and sanitizers. Pinkish biofilm residue inside the ice chute of the soda machine. Heavy accumulation of grease, carbon and static dust on the filters and in the housing of the hood vent. The hand-wash sink in the beverage area is being used as a food preparation sink as evidenced by staining of the basin and food debris in the strainer. Old unused equipment stored in shared hallway area should be removed from food facility. A working container of dish soap and stainless steel scrubbing pads were in the food prep sink.

The hand-wash sink in the food preparation was blocked by a cart and not accessible at all times for employee use; repeat violation, Dec. 15. Plumbing system not maintained in good repair: water leaking profusely at the faucet of the sink in the food preparation area between the walk-in cooler and freezer; repeat violation, Dec. 6 and 15. The floor throughout the back food preparation area and the dishwashing area is torn, bubbling and not smooth and easily cleanable. Lights are not shielded or shatterproof over the mechanical dishwasher.

Food facility has a reminder statement on the menu for foods that are/or could be served raw or undercooked to the consumer; however, does not clearly disclose to which foods the reminder statement applies. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration of mechanical warewashing. Dried food debris on inner blade and housing of electric slicer. Static dust on air return vents above food prep and food equipment storage.

The quaternary ammonia concentration in the sanitizing solution of a sanitation bucket in active use was 0 ppm, rather than 200 ppm as stated on the manufacturer’s use directions; corrected. Food facility is offering for sale foods from El Corazon Bakery (cheesecakes, rice puddings), which are prepared in an unapproved private home. Just Delta8 Red Drops Gummies and Nok Out Delta 8 + HHC + THCP Gummies contain an unapproved additive as specified in 21 CFR 170-180 relating to food additives. Such products are under U.S. Food and Drug Administration regulation and at which time CBD is not recognizable as a safe substance to be added to food. Raw shell eggs were stored above water in the walk-in cooler. Night crawlers stored on the same shelf with juice for slushies in the walk-in cooler.

Temperature measuring devices for ensuring proper temperature of equipment is not available or readily accessible in double-door reach-in and dining room cold hold equipment. Black biofilm residue on the housing gasket of the cuber on the interior of the ice maker.

The Golden Whisk Bakery, 245 Locust St., Columbia, Dec. 19. Pass. Food facility has an employee that has taken food safety training program; however, the food safety program was not an ANSI/CFP-accredited certified food manager program. The food facility has a person enrolled in the class for managers. The receiving door at the back of the food facility has a gap at the bottom and in the middle and does not protect against the entry of insects, rodents and other animals.
