The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

B&A Paradise Pizza, 700 Reinoel St. Lebanon, Follow-up, Dec. 15. Pass. Observed clean food equipment in storage area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Person in charge will work on improved training.

Buddy’s, 35 S. 8th St. Lebanon, Follow-up, Dec. 15. Pass. Observed one scored cutting board not resurfaced or discarded as required. A drawer full of clean food utensils, a food contact surface, was observed to have food residue and was not clean to sight and touch. Note: Person in charge stated that many of the utensils are no longer used in the facility. Obsolete items should be stored at another location so that cleaning can be completed properly and at the needed frequency.

Black Diamond Restaurant, LLC, 607 Cumberland St. Lebanon, Opening, Dec. 14. Pass. Potentially hazardous ready-to-eat food prepared in the food facility needs to be dated for a maximum of seven days. Note: adding six days to the date an item is made will give the correct date for the final date to be used or the date to be discarded. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Facility needs to obtain sanitizer prior to opening. Employee toilet room and women’s toilet room are not provided with a covered waste receptacle for sanitary napkins. Paper towel dispenser missing at the handwash sink in the prep area and the employee toilet room.

Angelo’s, 1032 S. 5th Ave. Lebanon, Dec. 13. Pass. Food employees observed in the food preparation area, not wearing proper hair restraints, such as nets, hats, or beard covers. Can opener blade, a food contact surface, was observed to have food residue and was not clean to sight and touch.

Hardee’s #1383178, 198 W. Lincoln Ave. Myerstown, Complaint, Dec. 13. Pass. The Person in Charge did not demonstrate adequate knowledge of the requirements of the food code as evidenced by the operating the the facility with an imminent health hazard present. Plumbing system not maintained in good repair – observed raw sewage coming out of the floor drain in the Men’s restroom when toilet was flushed and running water in the handwash sink.

Blue Bird Inn, 2387 Cornwall Rd. Cornwall, Dec. 12. Pass. Beef chili was held at 105°F, in a hot holding unit, rather than 135°F or above as required. Observed deeply scored cutting boards not resurfaced or discarded as required. Non-food contact surfaces (walk-in cooler fan guards) not cleaned at a frequency to preclude accumulation of dust.

Francisco’s Pizza, 139 E. Main St. Fredericksburg, Dec. 12. Pass. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. The interior of the ice machine, a food contact surface, was observed to have mold and was not clean to sight and touch.

Ideal Food Basket, 640 N. 7th St. Lebanon, Follow-up, Dec. 12. Pass. Raw chicken was stored above raw beef and/or pork in the walk-in cooler used by the prepared food department and in the walk-in cooler used by the meat department. At the time of the inspection, the meat department does not have available sanitizer test strips to determine appropriate sanitizer concentration. The drain line under the handwashing sink in the food service area was leaking at the start of the inspection. The drain under the 3-compartment sink in the food prep area was clogged with food debris. The water was running across the floor to another drain. An extension cord in the food receiving area was frayed in two spots. One spray bottle, a working container in the meat cutting area, used for storing chemicals/cleaners taken from bulk supplies, was not marked with the common name of the chemical.

Boyer’s Tavern, 285 Rexmont Rd. Lebanon, Dec. 11. Pass. Burgers and steaks are served undercooked to the customer’s request; however, a written consumer advisory (on the menu, table tent, or placard) is not provided to the consumer. Observed deeply scored cutting boards not resurfaced or discarded as required.

Holiday Inn & Express, 3 Everest Ln. Jonestown, Dec. 11. Pass. The handwash sink in the kitchen does not have single use towels, continuous towels, or air drying device.

Kugo Steak House, 1723 Quentin Rd. Lebanon, Dec. 11. Pass. Scoop being stored in the ice storage container with handle touching the ice. Observed wet wiping cloths in the kitchen, not being stored in sanitizer solution. Non-food contact surfaces (hood filters) not cleaned at a frequency to preclude accumulation of grease.
