Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed operator’s cellphone cord coming in contact with three knives on a prep table; corrected on site. Observed most, but not all, food ingredient storage containers, shakers, bottles in the entire kitchen area not labeled with the common name of the food. Food employees observed in kitchen cooking area not wearing proper hair restraints, such as hair nets or hats; corrected on site. Observed a broken chipped and bent knife blade being used to cut goat. Blade fragments were missing from the knife and could be in the food being cut. Operator stated that the company just brings him new knives every week, so he did not see an issue with this. Observed deteriorated gaskets on a nonworking refrigerated storge area. Gaskets must be removed or replaced to keep contamination (heavy food debris buildup) from spreading. Food dispensing utensils in numerous ingredient plastic storage containers observed stored in the food and not with handle above the top of the food and the container. No handled dispensing utensils (bowls, single use cups, etc.) observed stored in plastic ingredient storage containers throughout the facility. Observed wiping cloths in extremely unclean condition. Numerous food ingredient storage containers on shelving throughout the facility including the back dry stock area in the back area stored open with no covering. Observed a cut cucumber not wrapped or protected in the walk-in cooler. A grater with dried food debris on the equipment; corrected on site. A cut plastic wrapped cucumber (from the store wrap) stored on top of cut onions and peppers contaminating onions and peppers from human contact in the store of the plastic wrapped cucumber. Observed operator stepping directly on top of a food prep table with his shoed feet. Floor dough mixer in extreme dirty conditions with caked-on dried food debris on the entire machine including the guard. Manual table mounted can opener with dried food debris on the blade and not clean to sight or touch; corrected on site. Numerous dish racks with heavy buildup of dried food debris and not clean to sight or touch. Some not all pans in all bain-marie cooling units are not being changed out daily, as required. All spoons and ladles in food ingredient storage containers in the walk-in cooler are not being changed out or cleaned daily. Two plastic tubs of utensils at the dish area with accumulation of food debris inside of the tubs coming in contact with equipment stored. Old unused equipment stored in the area above the walk-in cooler area and throughout the facility should be removed from food facility. Observed all food items (commercially processed and food prepared in the food facility) in the walk-in cooler being held for more than 24 hours and not dated-marked. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Kitchen is in deplorable condition with large amounts of food debris on shelving, flooring, disorganization, chemical stored near food items, etc. and in need of reorganization and deep-cleaning. All large ingredient plastic storage containers with accumulation of dried food debris on the exteriors and in need of cleaning. The handles on all equipment (bain-marie, doors, etc.) all in need of thorough cleaning with heavy buildup of food debris. Wooden shelving unit in the back of the kitchen with accumulation of food debris on all shelving and in need of cleaning. Back dry storage area near the rear exit door in extreme disorganization and in need of cleaning. Cardboard is being used to line shelving. Ceiling tiles missing in the kitchen area and in need of replacement. Wet-wiping cloths are not being laundered daily. Observed single-service, single-use articles stored in entire kitchen area which were not in the original protective package or inverted to protect against contamination. Hand-wash sink is observed dirty and in need of cleaning to prevent contamination from splash. Observed numerous black flies in the back storage area near the rear exit door. Could not locate source as numerous sources were apparent (bags of onions, wet, not hung mops). Mops are not being hung to air-dry.
