Observed food employee touch raw beef with gloved hands and changed gloves without a proper hand-wash in between. Food employee observed in cooking area wearing watch on wrist. Observed two cases of raw eggs on rack in walk-in cooler stored above boxes of vegetables with potential for cross contamination. Facility has a certified food employee, but a certified food employee was not available during all hours of operation. Observed numerous food items (multiple bags of cheese, pasta prima, prepared foods, deli meats) in the walk-in cooler being held for more than 24 hours and not dated-marked. Sponges observed in the three-bay sink area being used to clean food contact surfaces; corrected on site. Observed the exterior bottom diamond-plated trim of the walk-in freezer as deteriorated and nonexistent with the wood behind it exposed. Diamond-plating is in need of repair/replacement. Observed four large and two small single-use aluminum pans being reused multiple times for food storage and/or preparation drying at the three-bay sink area; corrected on site. Manual vegetable slicer with dried food debris on the blades of the slicer. Covered slicer, which should be stored clean, with heavy dried food debris on the blade underside and not clean to sight. Two wall-hung magnetic knife holders with accumulation of food debris coming in contact with knives stored. Nine knives stored on both of these holders with dried food debris and accumulation of splatter from the slicer. A drawer at the main prep table on the cook line with food debris and opened packages of AA and AAA batteries coming in contact with the utensils stored inside. The electric knife sharpener with dried food debris on the exterior of the sharpener. Black plastic tasting spoons inside of a black cylindered container coming in contact with crumbs in the bottom of the container. Manual hand sharpener with dried food debris on the handle of the sharpener. A large salad spinner stored with dried food debris on the interior. Foil is being used to line a bottom rusted shelf in the equipment room. Foil is peeling and rusted shelf is exposed to equipment stored. Foil is not a stable cleanable surface. Ice scoop stored directly on top of a food box instead of in the designated holder. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Two drawers at the main cook line prep tables with heavy accumulation of dried food debris and in need of cleaning. Walk-in cooler floor with food debris and not clean to sight or touch. Exterior door, wall and floor of the walk-in cooler and walk-in freezer with heavy black mold-like substance and in need of cleaning. The top of a food chopper with food splatter, stored on a shelf under the slicer area. Exterior stainless lids, stored to the left of the main cook ovens on a green shelf, with heavy dust accumulation with the possibility to contaminate foods. The floor in the walk-in cooler with accumulation of paper and food debris and in need of a deep cleaning. Observed food equipment (plates, pitchers, etc.) stored in a back storage area directly on the floor and not 6 inches above the floor. Observed too numerous to count inverted glass bowls, plates, etc. stored on shelving in the back area with food debris and heavy static dust on the inverted side of the equipment and in need of cleaning and storing covered when not in use. Observed the hand-wash sink next to the three-bay sink area being used as a food preparation or dump sink as evidenced by observed food and a slimy build-up in the sink. Tile flooring, in the laundry room area, is missing several tiles and in need of repair. Observed cardboard being utilized as a floor covering in the walk-in cooler. Ceiling tiles are in need of repair in the dry equipment/catering storage room. Observed too numerous to count mouse-like droppings in various dry storage areas of the facility. Old unused equipment stored in numerous areas of the facility should be removed from food facility.
