Heat adjustment dials for stove/flat-top grill in cooking area with accumulation of old, encrusted food residue; corrected on site. Plastic shield around ice dispenser for soda machine in wait staff service area with accumulation of pink slime residue; corrected on site. Observed fan covers in walk-in cooler with large accumulation of static dust and need cleaning. Observed clean stainless steel food containers, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting).

Food employees observed prepping icing cups not wearing proper hair restraints, such as nets, hats, or beard covers; corrected on site. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Drawer in prep area with sugar/cinnamon residue containing smallwares; corrected on site. Observed old food residue and buildup in the prep sink in need of deep-cleaning. Observed standing water and debris in the bottom of the two-door refrigerator. Observed buildup and coffee stains in the hand-wash sink indicating uses other than hand-washing. Vents above the production area have static dust and are in need of cleaning.

Mikado Japanese Steakhouse & Sushi Bar, 4717 Carlisle Pike, Mechanicsburg, Type 2 follow-up, Oct. 30. Pass. Observed wood pallets being used on the freezer floor; wood pallets are not easily cleanable. The light intensity in the food preparation area is not at least 50-foot candles. Several bulbs in the prep appear to be going bad and/or missing.

All food employees observed in coffee area and the prep/cook/assembly area not wearing proper hair restraints, such as nets, hats, or beard covers. Observed the hand-wash sink in the back prep/assembly kitchen area with dirty cups, bowls and plates. Discussed with owner and corrected on site. Observed exit door in the back kitchen area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Owner will discuss with landlord.

McDonald’s No. 7928, 1 Lemoyne Drive, Lemoyne, Oct. 29. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Two food employees observed in prep/cook/serving areas not wearing proper hair restraints, such as nets, hats, or beard covers; corrected. Observed two ice chutes on the self-serve fountain soda dispensing machine with a brown mold-like substance on the interiors of the chutes, coming in contact with the ice dispensed; this could not be corrected on site. Four lid dispensers with accumulation of food debris and sticky residue on the interiors of the dispensers coming in contact with the self-serve lids. Observed the underside of the soft-serve dispenser with dried ice cream debris on the nozzle area and was not cleaned from the night before. Observed the exterior of the frozen Fanta machine with dried food debris on the entire side and in need of cleaning. The food facility does not maintain Food Employee Certification records as required. No certification was available to be observed. Observed exit door to the outside in the back of the store area of the food facility is being propped open; corrected on site.

Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low-temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Facility has three-compartment sink to wash, rinse, and sanitize all equipment and utensils. Detergent bulk container supplying detergent to the wash compartment of the mechanical warewashing equipment was empty; corrected on site.

Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed raw meat and poultry stored above ready-to-eat foods in the walk-in cooler with potential for cross-contamination: Raw beef stored above open case of lemons and raw eggs stored above deli meats. Observed wet-wiping cloths in prep/cooking area not being stored in sanitizer solution. Observed multiple boxes/containers of food stored directly on the floor in the walk-in cooler, rather than 6 inches off the floor as required. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed piping underneath hand-wash sink repaired with plastic wrap, which is not easily cleanable or approved for food equipment. Plates stored on shelf in cooking area with accumulation of food debris and splash residue. Microwave in cooking area with accumulation of food debris on interior surfaces and needs cleaning. Observed piping underneath hand-wash sink in kitchen area disconnected allowing water to flow directly on to the floor and needs repairs.
