Observed baked potatoes wrapped in foil overnight in the walk-in cooler. Observed numerous foods in all cooling units in the kitchen area stored open and unprotected underneath dirty racks with potential for contamination. Observed food stored directly on the dirty floor in walk-in cooler underneath racks, rather than 6 inches off the floor as required. Observed numerous food items (commercially processed and food prepared in the food facility) in all cooling units, and being held for more than 24 hours, is not dated-marked. Mechanical warewashing equipment observed with buildup of filth and food residue and needs frequent cleaning. Observed microwave in prep area damaged/peeling on top interior surface and is no longer durable for normal use in the food facility. Observed open container of homemade hot sauce on table in prep area with accumulation of old, encrusted food residue. Observed cooking/cold holding equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on nonfood contact surfaces. Observed cooking equipment in the kitchen area with encrusted grease and soil accumulation. Observed the following on nonfood contact surfaces: Floors in kitchen and walk-in cooler with accumulation of dirt/food debris and need cleaning. Food stored directly on the floor in food prep area, rather than 6 inches off the floor as required. Observed the following stored on the floor of the food prep area: One case of bananas, four cases of waffle mix, and three cases of paper napkins. Food facility does not have a valid food handler certificate posted. The hand-wash sink in the food prep area is blocked by and is not accessible at all times for employee use. Observed the hand-wash sink in the food prep area completely obstructed by a large garbage can; garbage can lid was resting/covering the hand-wash station. Observed the following: Paper towel dispenser empty at the hand-wash station in the food prep area; soap dispenser empty at the hand-wash station in the food prep area; an additional wall-mounted hand soap dispenser and related hardware observed in the basin of the hand-wash station. Plumbing system not maintained in good repair – observed a leak on the faucet of the three-bay sink. Observed poisonous or toxic material other than those required for the operation and maintenance of a food facility. Observed five LP gas tanks stored on the floor of the food prep area directly in front of the three-bay sink. LP gas is prohibited from being stored indoors at any time and is at risk of spontaneous ignition due to leak or rapid temperature change and should only be stored outdoors in a well-ventilated area away from direct sunlight.

Discussed with management and staff was instructed to not use the ice until maintenance could clean the machine; corrected on site. Observed wash temperature-measuring gauge on the mechanical dish machine not operating and only reading 140 F. Used facility temperature measuring plate to check temperature and machine did measure at 155.3. Management will contact technician to repair/replace gauge. Mechanical warewashing equipment does not have a manufacturer data plate with operation specifications. Observed static dust accumulation on the ceiling tile, grids and large floor fan in the kitchen area and in need of cleaning. Observed staining on the ceiling tiles in the kitchen area and in need of replacement. Observed food debris buildup on the exteriors of three blenders in the kitchen area and in need of cleaning. Observed upper interior of microwave oven, in the kitchen area, with food accumulation with the possibility to contaminate foods being prepared. Plumbing system not maintained in good repair – observed the stock/soup faucet leaking at the faucet and in need of repair. Observed the dish machine vacuum piping leaking onto the dish machine and in need of repair.

Observed dishes not being sanitized at the three-bay sink area. Observed three-bay sink not being set up as wash/rinse/sanitize and dishes were being washed from a bucket. Observed clean food equipment and/or utensils in the blue utensil container on the prep table in the three-bay sink area, stored uncovered or not inverted. Cooked yucca and container of corn was held at 92 F in the hot-hold box rather than 135 F or above as required. Employee tried to also place cooked chicken inside of this same hot-hold box. Owner turned on the oven and explained to employee to place hot foods inside of the oven. Corrected on site. The water temperature of the wash compartment of the manual ware-washing equipment was 72.3 F, rather than not less than 110 F, as required. Mops are not being hung to air-dry. Four spoons stored directly on top of dirty container lids on top of and inside of a bain-marie in the cook area; corrected. A small dirty knife placed on a table in front of the microwave oven; corrected. Four large knives stored dirty on two magnetic wall holders; corrected. Two dirty peelers, two dirty knife sharpeners, and a piece of dried food stored in the blue utensil basket on the prep table across from the three-bay sink area; corrected. Large clear plastic bin with lids stored inside coming in contact with accumulation of grease on the interior of the bin. Chicken skewers stored dirty in a plastic container stored on the table across from the three-bay sink area. Two rice cookers not cleaned from the night before, stored on table across from the three-bay sink area. Three sets of dirty gloves stored inside clean fryer baskets which were touching the wall above the three-bay sink. Three large chicken skewers stored on the floor in the cook area coming in contact with a bait trap; corrected.

Observed large bowl of raw chicken stored above ready-to-eat foods in the walk-in cooler with potential for cross contamination. Observed numerous foods stored open and unprotected with no covering in all cooling units in the kitchen and prep areas. Old food residue, dishes and utensils observed in the hand-wash sink in kitchen area, indicating uses other than hand-washing. Observed clean stainless-steel containers and bowls throughout the kitchen area, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Observed pest control device on dry ingredient storage shelf in back storage area with potential to contaminate foods; corrected. All food ingredient storage containers, in the kitchen area, are not labeled with the common name of the food. Observed door to the outside in the back prep area of the food facility is being propped open with potential for insects/rodents to enter the facility; corrected. Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by old/decaying rodents in pest control devices in the kitchen area on floor underneath cooking equipment and back storage area underneath dry ingredient storage racks. Floors throughout the entire kitchen area are extremely filthy and in need of cleaning. Observed boxes of food stored directly on the floor in the walk-in freezer, rather than 6 inches off the floor as required. Observed package of raw chicken thawing in standing water in the three-compartment sink, which is not an approved thawing method. Prep tables in kitchen area with accumulation of filth and are not being cleaned and sanitized frequently. Shelving in kitchen area storing plates, food ingredients, and food containers are extremely dirt with old food debris, food residue, and rodent droppings. Heat adjustment dials on cooking equipment in kitchen area have large accumulation of old, dried, food residue. Stainless steel and plastic containers on storage shelf in kitchen area being stored dirty and stacked together with clean containers. Hand-wash sink in kitchen area with large accumulation of filth and grime. Large handheld blender stored hanging with heavy accumulation of old food residue on blades. Stainless steel container storing dirty cooking utensils and knives with accumulation of food debris on interior surfaces. Large mixer in prep area with accumulation of old, dried food debris on food contact surfaces and is not being frequently cleaned and sanitized. Storage racks in all cooling units with accumulation of food debris/dirt. Containers storing numerous spices in prep area with accumulation of filth and are not being cleaned frequently. Food ingredient storage bottles in cooking area have large accumulation of filth and old, dried, food residue on nozzles and lids and are not being cleaned frequently. Multiple tables and booths in dining area have accumulation of food debris and food residue and need more frequent cleaning. Observed no available paper towels at the hand-wash sink in the kitchen area and the handwash sink behind the bar. Observed screen door to the outside in the back of the kitchen broken and hanging off the hinges and should be removed or replaced. Observed too many to count rodent droppings on floors and shelving throughout the entire food facility indicating obvious presence of rodent activity. Observed chemical spray bottle in dining area not labeled with the common name of the chemical.
