Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Multiple foods in all freezer and refrigeration units including but not limited to the following, pancake batter, mashed potatoes, onion rings, hot sauce, stored open with no covering. Employee was observed putting on dish-style gloves to mix potatoes without washing their hands. Dish gloves are not intended for use with food; corrected. Large bag of onions stored directly on the floor in prep area rather than 6 inches off the floor as required. Dirty floor mats rolled up and leaning on a shelf in contact with bagged bread. Approximately five to 10 wiping cloths in extremely unclean condition on a prep table. Refrigerated, ready-to-eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours in the both refrigeration units not being date-marked. Approximately 10-15 cucumbers were observed in the double-door reach-in refrigerator area were spotted with mold; discarded. One red cutting board and one green were not clean to sight or touch; both cutting boards had a large amount of old food debris from previous days use. Slicer had an excessive amount of buildup of old food debris from previous days use. Numerous dishes stored clean in a bus tub had food debris on them. Dishes were being stored under the grill. Waffle maker has old food debris covering the interior of the unit and has spilled on a sheet pan not cleaned from previous days use. Interior of the microwave had old food debris covering all walls and ceiling of the interior. No sign or poster posted at the hand-wash sink in the kitchen area to remind food employees to wash their hands. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Sides of fryer with a large accumulation of grease; under the grill has a large accumulation of grease and old food; waffle iron on a sheet pan covered in an extremely large pile of waffle debris; interior of all refrigeration units with debris and standing water on the bottom of each unit. Hash browns were held at 125 F in the kitchen area rather than 135 F or above as required; corrected. The hand-wash sink in the kitchen area does not have single-use towels, continuous towels, or air-drying device. Sauces and soup was held at 48 F in the Hobart double-door refrigerator rather than 41 F or below as required. All other items moved to a different unit for cold-holding. Discarded on site. Observed an egregious amount of grease and old food debris under fryer and flat top grill. The hand-wash sink was extremely dirty had yellow film buildup on the entire interior of the bowl.

Bella Nonna Italian Ristorante, 4700 Gettysburg Road, Mechanicsburg, change of owner, Sept. 30. Pass. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed multiple foods in the Avantco freezer in cooking area stored open with no covering; corrected. Floor in the warewashing area is made of tile and is cracked, roughened, and is not a smooth, easily cleanable surface.

Lower Allen Diner, 3449 Simpson Ferry Road, Camp Hill, Sept. 30. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed numerous food items in the walk-in cooler and the Avantco single-door upright freezer stored open with no covering. Observed a 24-piece carton of raw shell eggs stored directly on top of cheeses in the Avantco two door bain-marie cooling unit; corrected. Observed a red bus tub with panko on the interior with clumps from being used to dip raw foods into before cooking. Red bus tub was stored at room temperature inside of the stainless storage table on the back prep/cook line and not emptied and cleaned daily; corrected. Numerous foods (sauces, gravies, mashed potatoes, etc.) was held at 68.9 to 89.4 F in the hot-hold well area, rather than 135 F or above as required; repeat violation. Observed cellphones, charging cords and numerous drinks on food contact areas throughout the entire back kitchen area. Observed a hole in the stainless-steel countertop at the tabletop waffle maker with accumulation of food debris. Hole is not cleanable, and counter is in need of repair. Observed the lid of the Avantco two-door bain-marie cooling unit is broken and does not close or open correctly and is in need of repair/replacement. Observed food ingredient storage containers, bottles and shakers in the kitchen and wait staff areas not labeled with the common name of the food; repeat violation. Two silverware dish machine caddies in dish machine area are not in good repair or condition and in need of replacement. Food employees in kitchen area not wearing proper hair restraints, such as nets, hats, and beard covers; repeat violation. The wash solution temperature in the mechanical warewash equipment sanitizing with chemicals was 118 F, rather than not less then 120 F, as required. Ran the machine 25 times and could not get the gauge to read over 115 F, but probe thermometer read 118 F. Owner will call in for repair/check. Faucet and spray arm at the pre-wash area of the dish-machine with pieces of plastic on the spray arm and handles of the faucet areas. Plastic is causing faucet and spray arm to not be cleaned appropriately. Observed two cases of food stored directly on the floor in walk-in freezer area, rather than 6 inches off the floor as required. Tile flooring in the kitchen area was observed to be cracked, broken missing pieces and is not a smooth, easily cleanable surface. Observed the following on food contact surfaces: A faucet, no longer in working condition, on the wall directly over the hot-hold well (faucet was previously used to fill hot hold water) with accumulation of dried food debris directly over hot foods being held. A knife stored inside the Avantco two door Bain Marie top slot between the bain-marie containers and the holders coming in contact with heavy food debris not cleaned. Corrected on site. Two knives stored in the Avantco two-door Bain Marie not cleaned from the night before. Corrected on site. Foods not changed out daily, as required, in the Avantco two-door bain-marie cooling unit, as evidenced by dried food on the interiors of the stainless containers inside. A lid used to cover foods cooked on the grill, sitting directly on top of dried food debris used egg cartons. Numerous food bottles, shakers and containers are not being cleaned between fillings on the exterior and interiors. Used cardboard is being used to line shelving coming in contact with food contact surfaces. Shelving under cardboard is rusted and paint is chipping/peeling and in need of replacement or repair. Observed a hand-held peeler with accumulation of dried food debris on the blade area, stored on a shelf at the back prep/cook line stainless table. Corrected on site. Food chopper on the back cook/prep line with dried food debris on the blade and interior of the chopper. Manual Can Opener on the back prep/cook line with dried food debris on the blade and not clean to sight or touch. Two black and one gray bus tubs, stored on the bottom shelving at the dish machine, with dried food debris on the contents or inside coming in contact with the utensils, lids, etc. stored inside. Corrected on site. The three-arm milkshake machine with dried food debris on the underside, mixing blades and arms of the machine and not cleaned from the night before. Corrected on site. Soap was not available at the hand-wash sink in the back prep/cook line area. Corrected on site. Numerous foods were not heated before being placed in the steam table and not to 135 F for hot-holding as required. Walls and ceilings in the walk-in freezer area are not sealed to provide a smooth, non-absorbent, easily cleanable surface. Paint is peeling on all walls and ceiling. An insect control device (seven sticky fly strips) in entire kitchen ceiling areas with potential to contaminate food, equipment, and utensils; repeat violation. Paper towel dispenser empty at the hand-wash sink in the back prep/cook line area. Corrected on site. Observed all food items (commercially processed and food prepared in the food facility) in the walk-in cooler, and being held for more than 24 hours, is not dated marked with the date it was opened or made; repeat violation. Rear screen door located in the back of the kitchen area of the food facility has a gap at the bottom of the screen door and does not protect against the entry of insects, rodents, and other animals. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Observed the 10-burner gas stovetop on the back prep/cook line with accumulation of food on the burners and in need of cleaning. Observed the upper portion of the mechanical dish machine with accumulation of dried food debris and not clean to sight or touch. Two black plastic skids, in the walk-in cooler, being used to store product off of the floor, with heavy accumulation of dried food, paper and cardboard inside of the deep wells of the skids. A black mold-like substance on the caulking perimeter of the pre-wash area of the ware-washing machine and in need of cleaning and replacement of the caulking. Underside and back splash panel of the cappuccino machine with dried accumulation of static dust and food debris; corrected on site. Heavy accumulation of static dust on the underside of the hot hold expo line at the wait staff area. Heavy accumulation of static dust under the shelving of the expo line over clean plates used for service. Old unused equipment (two chest freezers, a floor model dough mixer, large meat saw and milk dispensing machine) stored in office, kitchen and wait staff areas, should be removed from food facility; repeat violation. Observed cardboard being utilized as a floor covering in the walk-in cooler and freezer.

Numerous foods (chicken, pork, beans, etc.) being held on the hot hold wells was held at 86.4 to 104.0°F, rather than 135°F or above as required. All food products were pulled and discarded. Corrected on site. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. Manager was observed touching his cell phone numerous times, and then touching food spoons and containers without a hand-wash. Food dispensing utensil in cheese, in the top portion of the bain marie cooling unit, observed stored in the food and not with handle above the top of the food and the container. A temperature measuring device for measuring manual ware-washing wash and sanitizing water temperatures is not available. Observed wet wiping cloths on the three-bay sink ledge, not being stored in sanitizer solution. Educated staffing, corrected on site. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in refrigerated equipment and hot hold box equipment. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed a large rack for canned foods, in back prep area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. The water temperature of the wash compartment of the manual warewashing equipment was 104.7°F, rather than not less than 110°F, as required. Educated and water was emptied, sink was re-filled and dishes were re-washed. Corrected on site. Observed single-service, single-use articles stored in front assembly area which were not in the original protective package or inverted to protect against contamination. Large knives, stored in a plastic cylinder, were stored with crumbs in the bottom of the container coming in contact with the knives food contact end. Corrected on site. Manual can opener observed with rust on the blade surface and in need of replacement. Two plastic tubs with plastic utensils for customer use were stored with crumbs in the bottom coming in contact with the food portion end of the to-go utensils. Observed clean food equipment and/or utensils (plastic cylinder container with spatulas and whisks) in back prep area, stored uncovered or not inverted.

New Beginnings Diner, 732 Market St., Lemoyne, Sept. 29. Fail. Some, not all, foods in the walk-in cooler and upright cooling units in the kitchen area stored open with no covering. Raw shelled eggs and containers of liquid eggs stored over ready-to eat deli meat in the two door Avantco bain-marie cooling unit. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed scoops stored in food ingredient containers and not removed every four hours, or daily for cleaning, as required. Observed numerous food items (commercially processed and food prepared in the food facility) in the walk-in cooler and upright cooling units, and being held for more than 24 hours, is not dated marked with the date it was opened or made. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Observed ceilings with static dust and cobwebs over food prep/cook areas and in need of cleaning.

Nonna Ilva, 85 W King St., Shippensburg, September 29. Fail. Food employees observed in the Prep and kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours in the Baine Marie unit and the walk-in cooler , is not being date-marked. Observed that there was not soap available at the handwash sink. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. A food employee was observed touching sugared pecans – a ready to eat food – with bare hands. Food dispensing utensil in the flour observed stored in the food and not with handle above the top of the food and the container. Observed raw shelled eggs stored above RTE Foods in the walk-in cooler.
