Aya Steakhouse, 235 S. Spring Garden St., Carlisle, Sept. 25. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed trays of raw eggs stored directly above buckets of duck and soy sauces in the walk-in cooler; corrected. Hand-wash sink in the rear food prep area nonfunctional and closed for use with plastic wrap. Operator demonstrated a major faucet leak when the sink is in use. Hand-wash sink in the side prep/dish area did not have a water temperature of at least 85 F. Operator stated the facility is having issues with the water temperature at this station due to plumbing issues. Operator instructed to use sushi prep area hand-sink and repair the two nonfunctioning stations. Loose and/or deteriorating rubber door gaskets observed on all cooling units in the facility. Observed clean dish racks stored on the floor in the dish-washing area; corrected. Refrigerated ready-to-eat, time/temperature control for safety foods prepared in the food facility and held for more than 24 hours not being date-marked. Observed numerous containers of in-house produced/processed foods without proper date-marking in the walk-in cooler. Hibachi service area of the food facility is extremely dirty, dusty and in need of cleaning. Observed old food, food residue and dirt under the hibachi cooking areas in the customer dining room. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Old food residue observed on all food prep tables, including the shelving and table legs in the food prep area. Plumbing system not maintained in good repair; observed the hand-wash sink faucet leaking in the food prep area.

Kanagawa Sushi Family, 41 W. Main St., Mechanicsburg, Sept. 25. Fail. Broken knife handle being held together with tape that had dried food debris covering the tape. Several cardboard boxes with utensils covered with a white flourlike substance. Mixer had old food debris on the parts; corrected on site. Observed chemicals/pesticides being stored in backroom area on shelving with food and food contact items; corrected on site. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed multiple food items throughout the facility are stored open with no covering; corrected on site. Raw shellfish was being stored above several ready-to-eat products; corrected on site. Person in charge not able to produce records showing that raw or partially cooked ready-to-eat fish has been frozen by the facility or approved supplier as required, before use or sale. Owner was unable to find letters of parasite destruction. Refrigerated, ready-to-eat, time/temperature control for safety food prepared in the food facility held for more than 24 hours is not being date-marked. Observed cardboard and newspaper being used to line shelving throughout the facility. Cardboard is an absorbent material and not easily cleanable. Observed deeply scored cutting boards not resurfaced or discarded as required.

KFC No. 50, 313 Lowther St., Lemoyne, Sept. 24. Pass. Loose/deteriorated rubber door gaskets observed on the walk-in cooler door. Walk-in cooler door observed without a latch that is working properly to keep door securely closed. Observed peeling paint on some sections of the ceiling, making the surfaces no longer cleanable. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following on nonfood surfaces with the possibility to contaminate foods being prepared in the facility: Accumulation of static dust on the ceiling vents, lights, cameras, outlets, wiring, etc., which is in need of a deep cleaning. Two large black air intake vents with dripping A/C water and very heavy static dust, over a food prep area, and in need of repair and deep-cleaning. Hanging plastic directions display sign between two breading stations with food debris on the plastic. Placement of the signage is not ideal and needs reconsideration.
