Observed food debris on the floor on the cook line from previous day. The bain-marie at the cook line had a significant of debris on the bottom and is in need of being cleaned.

Sweet Frog, 6416 Carlisle Pike, Mechanicsburg, follow-up, Sept. 5. Pass. Observed floors under all refrigeration units in the back room have old food debris and dirt buildup. Observed the following: interior of microwave had old food debris; waffle maker had a large amount of old food debris buildup and is in need of deep-cleaning; both corrected on site.

Multiple foods in the walk-in cooler are stored open with no covering; corrected on site. Observed chicken, dumplings and rice cooling at room temperature on the prep table, which is not a proper cooling method; corrected on site. Observed large sugar bag stored directly on the floor in prep area, rather than 6 inches off the floor as required; corrected on site. Refrigerated, ready-to-eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours in the walk-in cooler is not being date-marked; corrected on site.

Plaza Azteca, 5500 Carlisle Pike, Mechanicsburg, Sept. 3. Pass. Several raw animal foods were stored above ready-to-eat foods in the walk-in cooler; corrected on site. Observed single-service, single-use articles stored on the cook line and not inverted to protect from overhead containments; corrected on site. Observed two pairs of damaged tongs; discarded on site. Small amount of mold in upper wall of ice machine. Dried food debris on slicer blade; corrected on site.

Silantra Asian Street Kitchen, 3499 Market St., Camp Hill, Sept. 3. Fail. A temperature-measuring device for measuring manual warewashing wash and sanitizing water temperatures is not available. Manager’s beverage container was observed on a bain-marie cooling unit in the front cook area, a food preparation area; educated and corrected on site. Observed numerous food items (commercially processed and food prepared in the food facility) in the walk-in cooler being held for more than 24 hours and not dated-marked. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Manager was in charge until owner could arrive for final inspection violation discussion. Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Observed self-serve Pepsi machine leaking water (condensation) from the underside onto a table and caught in a stainless pan on the floor; discussed with owner and discussed other options to catch leaking water. Manager observed in cook/prep area not wearing proper hair restraints, such as nets, hats and beard cover. Temperature-measuring device for ensuring proper temperature of equipment is not available or readily accessible in hot-hold equipment. Observed single-service, single-use articles stored in the small exterior storage shed directly on the floor and not 6 inches above the floor. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed exteriors of sugar and cornstarch containers stored under a shelf and three white rolling ingredients bins (white rice, brown rice, flour) extremely dirty and in need of cleaning. Observed the large exterior storage shed with accumulation of debris on the floors under the shelving and in need of cleaning. Observed food ingredient storage containers, bottles, shakers in the entire main cook/prep line area not labeled with the common name of the food. Floor in the laundry room area is broken, warping and peeling and is not durable, smooth or easily cleanable. Observed clean food equipment on shelving above the three-bay sink area stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Observed two exterior sheds with gaps, openings, air vents and does not protect against the entry of insects, rodents and other animals. Food-dispensing utensil in sugar and cornstarch containers was observed stored in the food because there was no handle; corrected on site. Food facility has a certified food employee, but a certified food employee was not available during all hours of operation, as evidenced by an inability to talk with the certified food employee during this inspection. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Manager was sitting directly on top of a food prep table on them main line; corrected on site. A black mold-like substance on the four dispensers drink machine (not self-serve) on the underside dispensing area and dispensing push pedals. A pink moldlike substance on the self-serve Pepsi machine ice chute. A long cobweb hanging over the cook area. Magnetic wall-mounted knife hanger with accumulation of sticky residue, rust and food debris coming in contact with three knives stored; knives removed and taken to dish; corrected on site.

Tavern on the Hill, 109 Howard St., Enola, Sept. 3. Pass. No violations.

The Peppermill Family Restaurant, 1010 Wesley Drive, Unit 100, Mechanicsburg, Sept. 3. Fail. Observed two-door upright refrigerator in cooking area holding numerous foods at temperatures exceeding 50 F such as beef at internal temperature of 55.6 F. All foods being held in this unit were disposed of during this inspection. Observed crab, fish and rice being held at internal temperatures of 48.1 F, 43.4 F, and 45.6 F in the bain-marie unit in cooking area rather than 41 F or below as required; corrected on site. Observed refrigerated, ready-to-eat, time/temperature control for safety foods (deli meats and cheeses) prepared in the food facility and held for more than 24 hours in the walk-in cooler are not being date-marked. Loose rubber door gasket observed on walk-in freezer and needs replacement. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed dirty cardboard being utilized as a floor covering in walk-in freezer. Wall above steam table in cooking area with accumulation of food debris. Midea upright freezer with large accumulation of food debris/crumbs on racks and bottom surface. Storage area holding sheet pans underneath table in prep area with accumulation of dirt/food debris. Soap was not available at the handwash sink in the kitchen area. Observed numerous foods in all cooling units in the kitchen area stored open with no covering. Observed multiple boxes/containers of food stored directly on the floor in the walk-in freezer, rather than 6 inches off the floor as required. Microwave in cooking area with accumulation of food debris on interior surfaces; corrected on site. Heat-adjustment dials for stove burners with accumulation of encrusted food residue; corrected on site. Plastic container storing souffle cups/lids with accumulation of dirt/debris; corrected on site. Automatic slicer with accumulation of old food debris on blade and food-contact surfaces. Observed two spray bottles in the kitchen area storing chemicals for cleaning not labeled with the common name of the chemical; corrected on site.

Two Cousins Pizza & Restaurant, 32 Erford Road, Camp Hill, Sept. 3. Pass. No violations.

University Lounge and Grill, 32 King St., Shippensburg, Sept. 3. Pass. Food-dispensing utensils stored in the food and not with handle above the top of the food and the container.

Wendy’s, 601 Enola Road, Enola, Sept. 3. Pass. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Blood spots were observed on the floor in the walk-in freezer. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Old food debris was observed on the bottom of the upright freezer where breaded foods are stored. Insect light trap was saturated with flying insects. Glue board needs changed out more frequently. Facility needs to close rear external door while food orders are being delivered.
