No invoice or other documentation maintained by the food facility to confirm that whole-muscle intact beef is being used. Food Facility is using/offering for sale foods prepared in an unapproved private home. Packaged raw meat & poultry used or offered by the food facility is not labeled with safe handling instructions. Food Facility is unable to verify that fish being used or offered for sale is from approved sources.
