The water temperature of the wash compartment of the manual warewashing equipment was 107.4 F, rather than not less than 110 F, as required. Utensils, spatulas, scrapers, lids, ladle stored in a plastic bin coming in contact with dried food debris inside of the bin. Bin was under the prep table in the back kitchen area. Ceiling in the kitchen area is cracked, peeling paint, warping, roughened and is not a smooth, easily cleanable surface.

Drains throughout the facility have a buildup of black moldlike substance and food debris in need of cleaning. Warewashing machine has a large amount of buildup of grease, lime and food debris. Three-bay sink and two-bay sink have grease buildup on interior walls. Observed the following: buildup of white and black substance and old food debris on dish racks; exterior of numerous bus tubs with grease and food debris stored clean; bain-maries with excessive food debris on interior and mold on gaskets; exterior of food storage containers throughout the facility with old food debris and buildup. Observed clean dish racks stored on the floor under the dish machine; corrected on site. Overhead vents and ceiling tiles have static dust and debris, a potential contamination to food, in need of cleaning.

Two freezers with tape being used as a repair for gaskets and broken plastic. Between and under all equipment on the cooking line there is a buildup of old food debris and grease. The table to the right of the bain-marie including food-contact items needs to be cleaned. All refrigerator/freezer units have a buildup of old food debris on handles. Soda dispenser has buildup of pink moldlike substance on the underside of dispensers. The entire dish area is in need of deep-cleaning including the drain, chemical buckets, walls and floor. Observed buildup of black substance in the slop sink and drain. Observed a large amount of debris on the floor in the back of the facility in the dry storage area. Old food and greasy buildup on the chemical buckets. Walls and floor also have old food debris in need of cleaning.

Observed the drain at the dishwasher to be extremely dirty and noticed a foul smell around the dish machine drain believed to be coming from the drain. Paper towel dispenser empty at the hand-wash sink on the cook line. Old unused equipment stored in the dry storage area should be removed from food facility. No sign or poster posted at the hand-wash sink on the cook line to remind food employees to wash their hands. Screen door to the outside is not self-closing and/or is being propped open; corrected on site.
