Asian International Bazar, 3300 Hartzdale Drive, Suite 115, Camp Hill, July 30. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Observed numerous foods, cooking vessels and equipment, dishes, etc. all stored within the facilities prep areas and separated or labeled as such from facilities equipment and food. Observed food ingredient storage containers, bottles, pitchers, in the bain-marie prep area not labeled with the common name of the food. Observed wiping cloth in extremely unclean condition in three-bay sink area; facility owner removed and discarded. Observed potatoes stored directly on the floor in produce area, rather than 6 inches off the floor as required; corrected on site. Two pieces of meat in meat display case were internally temped at 47.3 and 54.1 degrees at this inspection; discussed with owner; meat is to be discarded. Observed a partial gallon of expired milk in the bain-marie cooling unit; discarded and corrected on site. Sponges observed in the three-bay sink area being used to clean food contact surfaces.; discussed with owner who stated they were for personal use; discussed that this is a place of business and must follow regulation; sponges were removed and discarded. Temperature-measuring device for ensuring proper temperature of equipment is not available or readily accessible in meat display case. Observed a round wooden butcher block being used to chop meats. Butcher block was chipping and stored dirty. Interior of microwave with accumulation of food debris and in need of cleaning. Yellow cutting board was stored dirty behind the meat display case area. Interior of bain-marie cooling unit with a black mold-like substance on the interior lid and back under the lid when it is opened. Two dirty and empty containers with spoons inside were observed stored in the bain-marie cooling unit. Observed a white residue spilled on the exterior of the meat display case and in need of cleaning. Observed single-service, single-use articles stored in bain-marie area, which were not in the original protective package or inverted to protect against contamination. Trash receptacles within the food facility that are not in immediate use are not covered properly. Trash receptacles are not being emptied daily, as required. Exit door to the outside, located in the back of the store of the food facility is being propped open; corrected on site. Food facility inspection indicates evidence of insect (small black flies/gnats and ants) activity in food areas, but facility does not have a pest control program. Mops are not being hung to air-dry. Food facility did not submit a HACCP plan for approval as required. Instructed owner to stop process until HACCP plan was submitted.

Little Caesars, 836 Market St., Lemoyne, July 30. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Employees were not trained on food safety. Employee observed changing tasks that may have contaminated hands without a proper hand-wash in-between; educated employees and corrected on site. Employee observed donning single-use gloves without a prior hand-wash; educated employees and corrected on site. An open employee’s beverage container was observed on the stainless prep table, a food preparation area; discussed with employees and corrected on site. Food employees’ purses, phones, drinks, food, etc., were stored on shelving and in walk-in cooler cart/shelving and not separated from facility foods. Employee paper plates, partially eaten foods, a coffee pot, etc., on the stainless prep table area; discussed with employees and corrected on site. Food employees observed in food prep/cook area not wearing proper hair restraints, such as nets and hats; discussed with employees and corrected on site. Observed food employee packaging ready-to-eat tomato sauce with bare hands and coming in contact with the inside of the cups and the interiors of the lids. Observed employee putting small pizza boxes together without gloves; bare hands were coming in contact with interior food contact area of boxes; discussed with employees and corrected on site. Several food items (tomato sauce, raw pizza with pepperoni) in the walk-in cooler area stored open with no covering. Observed food ingredient storage containers, bottles and shakers in the entire facility, not labeled with the common name of the food. Food brush used to brush pizza crust in the pizza prep area observed stored in a container of water not maintained at 135 F; discussed with employees and corrected on site. Observed both clean and wet-wiping cloths in extremely unclean condition in three-bay sink area and throughout the facility. Observed a case of dough mix stored directly on the floor in the walk-in cooler area, rather than 6 inches off the floor as required. Observed numerous spice shakers throughout the facility not being cleaned at a frequency to avoid buildup on the containers, especially the flour shakers. An unclean box of single-use towels was stored directly on the food contact portion of the EBECO pizza maker. Observed a box fan, stored on a box on top of a trash receptacle, with static dust on the fan guard and blowing directly on food contact and food areas of the kitchen. Observed numerous food utensils not being cleaned every four hours as required. Observed facility telephone placed directly on the food contact side of the pizza boxes on the pizza counter; discussed with employee and corrected on site. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Large pizza oven/conveyor with heavy accumulation of food debris on the catch trays and in need of a deep-cleaning. A black mold-like substance observed on the backsplash above the entire length of the three-bay sink and in need of cleaning or replacement. Observed the drainpipe and the drain from the three-bay sink with heavy black mold-like substance on the piping and drain and in need of a deep cleaning. Observed single-service, single-use articles stored on a shelf at the boxing area which were not in the original protective package. Cardboard boxes and black plastic to go containers were stored on the top shelf not inverted to protect against contamination. Observed clean food equipment on a shelf above the three-bay sink area, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Plumbing system not maintained in good repair – observed leaking at the faucet connection and is partially taped to try and correct leak. Old unused and broken equipment stored in entire facility, should be removed from food facility.
Burger King No. 554, 3555 Gettysburg Road, Camp Hill, July 29. Pass. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Accumulation of static dust on the back and wiring of all monitors. Accumulation of static dust on ceiling tiles over food prep areas in the kitchen area. Accumulation of static dust on ceiling tiles in both restrooms. Ceiling tiles and grids in the kitchen area is rusted and stained and is in need of replacement.

Karns Quality Foods No. 3, 4870 Carlisle Pike, Mechanicsburg, July 28. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Shellstock tags are not maintained for 90 days from the date the container is emptied; tag was missing for oysters on display. Grind log not available; educated team on proper procedures and maintaining of grind logs for beef. Two walk-in freezers have a large amount of ice buildup that has accumulated on the exterior boxes of frozen food items. Both freezers are in need of repair. The following food contact surfaces were observed: Several containers of small wares in deli with old food debris; meat slicer with dried food debris on the blade and guard; cutting boards with moldlike substance from being stored on the table with standing water in the seafood department; meat trays with a large amount of chipping in need of being replaced; tracks on the service cases have an accumulation of old food debris. Dishes and utensils observed in the hand-wash sink in the meat/seafood department, indicating uses other than hand-washing; corrected on site. Slop sink area of the facility is extremely dirty with black buildup on the walls, shelf and drain sink, including the drain. Back room ceiling has a large amount of dust and cobwebs accumulation throughout. Paper towel dispenser was empty at the hand-wash sink in the deli and meat/seafood area; corrected on site. Old unused equipment, supplies, etc., stored in back area; should be removed from food facility.
Neato Burrito, 6391 Carlisle Pike, Mechanicsburg, July 28. Pass. Soda machine ice chute had food residue and was not clean to sight and touch; corrected on site.
Sbarros Pizza, Capital City Mall Drive, Camp Hill, July 28. Pass. A food employee was observed touching cooked pizza with bare hands; discussed with manager and corrected on site. Observed interior bottom and top interior of lid and surrounding of bins of bain-marie on the pizza station with accumulation of food debris and in need of a deep cleaning. Observed static dust on the venting in the walk-in cooler and the interior of the venting with the possibility to contaminate foods stored. Observed hand-wash sink in the front serving area with food debris and staining and in need of cleaning; corrected on site.

Observed two plastic bins with utensils stored inside (in the back three-bay sink area) and one slotted utensil storage in a drawer (in the front prep line) with food debris in the containers coming in contact with food contact utensils stored inside; discussed with management and corrected on site.
Taco Bell, 3506 Capital City Mall Drive, Camp Hill, July 28. Pass. A temperature-measuring device for measuring manual warewashing wash and sanitizing water temperatures is not available; discussed with management. The water temperature of the wash compartment of the manual warewashing equipment was 81.8 F, rather than not less than 110 F, as required; discussed with management.
Tropical Smoothies Cafe, 3601 Market St., Camp Hill, follow-up, July 28. Pass. Observed splatter on the ceiling in the front smoothie area; repeat violation. Observed static dust on the wiring of the monitor at the sandwich/wrap bain-marie cooling unit with the possibility of contaminating foods being prepared below. Plumbing system not maintained in good repair – observed ice machine leaking from the underside of the machine. Floor tiles at the customer register area are broken and missing and is not a smooth, easily cleanable surface.
