Arby’s No. 1172, 32 32nd St., Camp Hill, follow-up, July 19. Fail. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Grease leaking from the fryers in the front cook area and heavy accumulation of grease behind and under the fryer units; repeat violation. A black mold-like substance was observed on the backsplash of the wall above the three-bay sink; repeat violation. Observed the following on food contact surfaces: bain-marie parts at the front assembly/cook area were not removed and cleaned from closing the night before, accumulation of food debris and dried food; repeat violation. Buttons on the roast beef slicer had dried food debris accumulation, not clean to sight or touch; repeat violation. A reused cardboard box with food contact bags stored inside with food debris coming in contact with the bags stored; cardboard is not a cleanable surface; corrected on site; repeat violation. Observed grease trap, flooring and large holding tank, flooring and surround walls with heavy accumulation of grease and in need of a deep cleaning and repair of wall area behind the holding tank; repeat violation. Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions and did not demonstrate adequate knowledge of food safety as evidenced by this noncompliant inspection. Wall in the back corridor area is observed broken and in need of repair; repeat violation. Cove molding on both sides of the walk-in freezer and adjacent to the walk-in freezer is observed missing, crumbling and in need of repair/replacement; repeat violation.

Theo’s Diner, 8 S. High St., Newville, July 17. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Commercially processed refrigerated, ready-to-eat, time/temperature control for safety food in the refrigerator, and held more than 24 hours, is not being marked with the date it was opened. Refrigerated, ready-to-eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, in the baine-marie and upright refrigerator, is not being date-marked.
Tropical Smoothies Cafe, 3601 Market St., Camp Hill, July 17. Fail. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. The following areas of the food facility were observed to be dirty and otherwise in need of cleaning: syrup residue on the ceiling above the smoothie station in the food prep area; dust/syrup residue on the ceiling mounted HVAC vent in the main food prep area; black, mold-like substance on the splash wall of the three-bay sink in the dishwashing area. Observed the following foods date-marked by the facility but beyond marked date and requires discarding: two containers of sliced bananas in the walk-in cooler; container of blueberries on the prep bain-marie unit in the main food prep area; product disposed of and corrected on-site. Observed cardboard being used as a shelf lining in the main food prep area. Can opener blade, a food contact surface, was observed to have food residue and was not clean to sight and touch. Can opener in the rear prep area; corrected on-site. Observed cardboard being utilized as a floor covering in the main food prep area. Cardboard is being used to control a ceiling leak in the area. Observed the roof leaking in the following areas of the facility: main food prep area directly above a food prep table; above the wall where licensing is displayed in the main food prep area; above the customer line leading up to the POS system in the dining area; repeat violation. Owner to repair roof leak.

Observed two containers of raw chicken being held at internal temperatures of 55.9 F and 52.6 F in the Turbo Air refrigerator, rather than 41 F or below as required; corrected on site. Observed cooked bacon in Turbo Air refrigerator being stored on empty, cardboard flats that held raw eggs and could have potential for cross-contamination; corrected on site. Observed visible evidence of rodent-contaminated foods in the back prep area, such as rodent droppings in large plastic container storing bag of onions; bag of onions was disposed of, corrected on site. Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by dead mouse in pest-control device in the back storage area; corrected on site. Observed clean food equipment (stainless steel food containers) on storage rack in ware-washing area, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Observed loose/deteriorated rubber door gaskets on the Turbo Air refrigerator in back storage area and needs to be replaced. Observed water coming up from grease trap on floor near cooking area and flowing into a floor drain nearby. Plumbing system not maintained in good repair. Observed water leaking from PVC piping below three-compartment sink. Observed the following on food contact surfaces: handles for hand-wash sink with accumulation of old food residue; lid for bus tub storing flour with accumulation of old, caked-on food residue; corrected on site. Floor in the kitchen area is made of tile and is cracked, roughened and is not a smooth, easily cleanable surface. Potential rodent harborage areas inside the food facility observed throughout the entire facility due to numerous rodent droppings on floors.
