KFC/Taco Bell, 3 Airport Road, Shippensburg, July 10. Pass. Food employee observed storing clean food equipment while wet and not allowing time for draining and/or air-drying.

Charleys Philly Steaks (@walmart), 6520 Carlisle Pike, Suite 550, Mechanicsburg, Type 2 follow-up, July 9. Pass. No violations.

Chen’s Asian Restaurant, 310 N. Hanover St., Carlisle, July 9. Fail. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed frozen raw food items being stored in the freezer in unapproved grocery style bags. Many of the products had ice crystals and were not protected. Observed the following items stored directly on the floor in kitchen area, rather than 6 inches off the floor as required: raw onions (corrected on site), packaging. Person in charge not able to produce records showing that raw or partially cooked ready to eat fish has been frozen by the facility or approved supplier as required, before use or sale. Observed items throughout the facility, to numerous to count, held for more that 24 hours, not properly date marked. Observed the following food contact surfaces not clean to sight or touch: three knives (corrected on site), one peeler (corrected on site), multiple storage containers with buildup. Ice machine had mold on the deflector plate. Observed the following nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: exterior of food storage containers and shelves throughout the facility. A dish was observed in the handwash sink, indicating uses other than handwashing. Corrected on site. No sign or poster posted at the handwash sink in the prep area to remind food employees to wash their hands. Old unused equipment, cardboard, unused supplies stored throughout the kitchen area, should be removed from food facility. Observed employee prescription and nonprescription medications being stored on shelving near food/equipment. Corrected on site. Employee personal items including but not limited to phones, wallets, and food items were observed throughout the entire kitchen area and not stored in identified storage areas as required.

Mt. Fuji Restaurant, 149 N Hanover St., Carlisle, July 9. Fail. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Raw chicken eggs being stored above ready to eat vegetables in the walk-in cooler. Observed food ingredient storage containers, throughout the facility, are not labeled with the common name of the food. Observed avocados and onions stored directly on the floor in kitchen area, rather than 6 inches off the floor as required. Person in charge not able to produce records showing that raw or partially cooked ready to eat fish has been frozen by the facility or approved supplier as required, before use or sale. Shrimp was observed thawing in standing water in the prep sink, which is not an approved thawing method. Corrected on site. Refrigerated, ready to eat foods, prepared in the facility being held for more than 24 hours with no date marking was observed throughout the facility. Foods are served raw or undercooked to the customer’s request; however, a written consumer advisory (on the menu, online menu, table tent, or placard) is not provided to the consumer. Observed cardboard being used to line shelves, that was extremely dirty. Cardboard is not smooth and easily cleanable surface. The following food contact equipment was observed to have food residue and was not clean to sight and touch: drawer containing knives, can opener, vegetable peeler; cutting board stored on the floor with food debris; dish racks (black grease-like buildup). Observed the following nonfood contact surfaces: fryer with a large amount of grease accumulation and build up, frying pans with extreme build up on the exterior, bain-marie has buildup of old food on the interior of the unit including the gasket, handles of drawers and refrigeration units have a build up of food residue, heavy grease build up on the pipes of the hood system and piping of cooking equipment on the cook line. A two-bay prep sink was identified as the designated handwash sink by the owner. One must be identified with signage stating for handwashing only and requires soap and either single use towels or air-drying device. Old unused equipment/supplies/tools and cardboard should be removed from the electrical room of the facility.

Newville Diner, 399 Carlisle Road, Newville, July 9. Fail. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Observed fish and chicken in the walk-in cooler and walk-in freezer stored open with no covering. Observed raw shell eggs stored on top of cheese in the baine-marie. Employee repeatedly on their phone, opening and closing the doors to the cooler, and then do food preparation tasks without changing gloves. Observed beef, chicken and produce stored directly on the floor in walk-in cooler and freezer area, rather than 6 inches off the floor as required. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the Baine Marie and walk-in cooler, and held more than 24 hours, is not being marked with the date it was opened. Observed a bucket of steak knives and kitchen utensils in the kitchen area with numerous dried food particles mixed in with them. The microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch on the inside top, bottom and sides. Observed four microwaves, in kitchen area, with an accumulation of food residue and debris on nonfood contact surfaces. An insect control device located in kitchen area with potential to contaminate food, equipment, and/or utensils. The back door to the outside, located in the prep/warewash area of the food facility is being propped open. Observed too many to count black and green flies in the kitchen, prep, dining and storage area. Observed the ceiling vents in the kitchen area of the food facility is extremely dirty, dusty, and in need of cleaning.

The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed peppers in the walk in freezer area stored open with no covering. Observed salt, pepper and garlic powder in food ingredient storage containers, in the kitchen area, not labeled with the common name of the food. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the walk in cooler, and held more than 24 hours, is not being marked with the date it was opened. Pizza Hut, 3300 Hartzdale Drive, Suite 113, Camp Hill, July 8. Fail. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed a food employee changing tasks that may have contaminated hands without a proper handwash in-between. Observed food employee serving customers at the point-of-sale and handling cash without washing hands before handling food as well as donning single-use gloves without a prior handwash. Employee cellphone and drinks observed on a prep table in the rear food prep area adjacent to open to-go food containers. Observed pizza sauce in the walk-in cooler stored open with no covering. Observed 15+ pizza crusts with a discard date of end-of-day Monday, July 7, ready for use in the walk-in cooler. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed a mold-like substance on the top of the gasket of the Traulsen refrigerated unit in the main food prep area. Soap was not available at the handwash sink in the main food prep area. Observed the hand soap dispenser in the food prep area to be inoperable due to dead batteries. Food employees instructed to use the hand wash station in the rear food prep area until rectified. Observed the following areas of the food facility extremely dirty, dusty, and in need of cleaning: accumulation of static dust on the ceiling and ceiling vents throughout the facility; dirt and grease buildup behind the pizza prep line, including the floor, in the main food prep area; dirt and grease buildup around the dishwashing area; static dust/mildew-like substance on the fan housing inside the walk-in cooler.

The person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. They did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Food facility does not have a HACCP plan for processing acidified sushi rice as required.

Sheetz Store #191, 1098 Harrisburg Pike, Carlisle, July 7. Pass. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Chlorine sanitizer ppm tested by both the myself and the operator. Operator placed emergency work order for repair and instructed to use the manual three-compartment sink for all dishwashing needs.
