Linglestown Diner, 5000 Linglestown Rd Ste 6 Harrisburg, Follow-up, June 27. Pass. Accumulation of grease and dust on the range hood, which is dripping on the cooking area below and contaminating food being prepared.

Boba Addict, 20 S Union St Middletown, June 26. Fail. A temperature measuring device for measuring manual warewashing wash and sanitizing water temperatures is not available. Observed no hot water available to properly wash hands. Close by Order. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Observed a large bag of non-diary creamer and a box of boba crystals stored directly on the floor in back curtained prep area, rather than 6 inches off of the floor as required. Loose/deteriorated rubber door gasket observed on the Avantco two door baine marie cooling unit. Gasket was barely attached to the door when opened. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following on non-food contact surfaces: Flooring behind the prep counter with heavy accumulation of food debris and not clean to sight or touch. Drains under dump sink and three bay sink with heavy accumulation of food build-up and in need of cleaning. Interior of the Black and Decker microwave oven in the curtained back prep area with accumulation of dried food debris. Facility does not have written procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility. The water temperature of the wash compartment of the manual warewashing equipment was 60°F, rather than not less than 110°F, as required. No hot water available to properly wash dishes. Employee was observed washing dishes without hot water or proper set up of three bays sinks wash/rinse/sanitize. Manual warewashing equipment (all three bays) observed with build-up of filth and food residue and not cleaned before use, and frequently throughout the day. The handwash sink in the front prep area being used as a food preparation sink as evidenced by observed staining from tea in the sink. Sponges observed in the three bay sink area being used to clean food contact surfaces. Observed the following on food contact surfaces: A black mold-like substance on all eight (8) juice pump dispensers in the back room of the prep area. A black mold-like substance on the interior hosing of the ice machine. Square Frying Pan and Round lid stored on top of induction plate with heavy food debris. Two (2) plastic containers, stored on top of the ice machine with ice scoops store inside are not cleaned daily, as required, as evidenced by food debris inside plastic containers coming in contact with the scoops stored. Observed single-service, single-use articles (cases of cups and lids) stored in the back curtained prep area and the back corridor near the exit door area directly on the floor, and not 6 inches above the floor. Observed door located in the rear exit area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. The handwash sink located in the front prep area does not have water at a temperature of at least 85°F. Close by Order. A large accumulation of empty boxes was observed stored in the large game room and need removed from the facility. Paper towel dispenser empty at the handwash sink in the restroom area.

Bruno’s Corner Bar, 101 Main St Middletown, Follow-up, June 26. Pass. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Facility will wash dishes/equipment by hand until machine is fixed.

Dairy Queen, 1651 S Cameron St Harrisburg, June 26. Pass. Observed wet wiping cloths in kitchen area, not being stored in sanitizer solution. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Accumulation of grease and dust on the range hood, which is dripping on the cooking area below and contaminating food being prepared.

Eatery @ Hollywood Casino, 777 Hollywood Blvd. Grantville, June 26. Pass. Food employee observed in cooking area, wearing watch on wrist. Corrected on site. Observed 3 bins storing cooking utensils on storage racks in back prep area with accumulation of old food debris on bottom interior surfaces. Corrected on site. Observed cooking equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces.

Harry Row Swimming Pool, 1 Park Dr. Halifax, Complaint, June 26. Pass. No violations.

Middletown Swim Club Bar, 1100 N Union St Middletown, June 26. Pass. Hot Dogs, acommercially processed food was only heated to 100 to 107 °F before being placed in the hot dog roller and not to 135°F for hot holding as required. Educated and corrected on site. Hot Dogs food were held at 100 to 107 °F, at the hot doller roller warmer area, rather than 135°F or above as required. Educated and corrected on site. Observed two (2) broken handles on the two white Galaxy chest freezers in the back storage area and are in need of repair. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following on non-food contact surfaces: Underside of the Mocha/French Vanilla machine with accumulation of sticky residue and not clean to sight or touch. Umderside of Nacho Cheese dispenser with splatter of dried cheese and in need of cleaning. Interior of Chefman oven with accumulation of crumbs.

Penn Drafthouse @ Hollywood Casino, 777 Hollywood Blvd. Grantville, June 26. Pass. Observed too many to count fruit flies around soda gun nozzles in the bar area.

Sheraton Harrisburg/hershey, 4650 Lindle Rd. Harrisburg, June 26. Pass. Observed buckled and loose metal floor panels in the walk-in cooler. Observed build-up of old food residue, dirt and debris in the floor drain adjacent to the three bay sink in the dishwashing area. Observed aluminum foil being used as a shelf covering above the range oven on the service side food prep line. Aluminum foil observed to be torn and is no longer an easily cleanable surface. Wash cycle temperature gauge observed to be not functioning on the mechanical warewashing machine in the dish area. Operator stated parts are on order; temperature verified as in the compliance range. Observed old food residue build-up on the gaskets of the salad bain-marie unit on the service line in the main food prep area.

Take Five Employee Cafeteria @ Hollywood Casino, 777 Hollywood Blvd. Grantville, June 26. Pass. No violations.

Champions, 300 S 2nd St Highspire, June 25. Pass. Old unused equipment stored in Kitchen and two dry storage area, should be removed from food facility or stored clean and cover. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed deeply scored cutting boards not resurfaced or discarded as required. Two boxes of French fries and two boxes of chicken tenders were observed stored in containers that had originally held cod, located on the door of the chicken freezer. This issue was addressed immediately and corrected with the manager. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following on non-food contact surfaces: -Observed gaskets on one door upright Hoshizaki Freezer with black mold-like substance. -Accumulation of grease and dust on the range hood, which is dripping on the cooking area below and contaminating food being prepared. -The walk-in freezer storage area exhibits significant disorganization and an accumulation of dirt, being unclean both visually and to the touch. -The floor of the walk-in cooler exhibits a significant accumulation of dirt and is not clean to sight or touch.

Golden Dragon, 6053 Allentown Blvd Harrisburg, June 25. Pass. During the inspection, two trays of raw chicken wings and one tray of scallops were found uncovered in the kitchen freezers. This issue was immediately discussed with the owner, and correcting on sight. Observed food ingredient storage containers and bottles, through-out the facility, is not labeled with the common name of the food. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Accumulation of grease and dust on the range hood, which is dripping on the cooking area below and contaminating food being prepared.

Mcdonald’s #1123, 2270 Harrisburg Pike Middletown, June 25. Fail. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed five spoons in the coffee area observed stored in a container of water which is not maintained at 135°F. Educated Management and corrected on site. Observed the following on food contact surfaces: Observed a black mold-like substance on the exterior dispensing and surrounding areas of the frozen drink dispenser. Management stated a company comes in to clean this machine. Frequency of cleaning needs to be updated to accomodate mold-like build-up. Cone dispenser with crushed cone crumbs/dust on the interior and not being cleaned daily to prevent contamination. Management did not wash/rinse/sanitize cone tray before trying to place back in dispensing slot. Discussed with management in reference to training. Frappe machine interior dispensing area with heavy build-up of food and not cleaned often enough to avoid accumulation. Air Conditioning is not working in the facility and two black counter fans were observed with static dust debris on the fan guards blowing over, and at, food and food contact surfaces. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following on non-food contact surfaces: Exterior of soft serve ice cream machine with accumulation of splatter and not cleaned from the night before. Exterior of Frappe machine with heavy residue on the upper exterior behind dispensing area and on the sides of the exposed areas of the machine. Underside of Sure Shot cream dispensing machine with accumulation of dried food debris. Interior and gaskets of the two door Beverage Air cooling unit with heavy crumbs and not clean to sight or touch.

Sam’s Fresh Mart, 4175 State Route 209 Elizabethville, June 25. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed sheet pan of cake in refrigerator labeled with “expire June 21st” and held past the discard date. Discarded and corrected on site. Observed ziploc bag of shredded cheese and bag of sliced deli meat with no labeling or datemarking in refrigerator. Discarded and corrected on site. Numerous ziploc bags of sliced deli meat (lebanon bologna, sweet bologna, turkey, & ham) observed in refrigerator with date of freezing, instead of date of thawing or opening. Operator was not able to identify date of thawing or if food items were being stored past seven days of thawing due to noncompliant date-marking. Discarded and corrected on site. Observed two three-drawer plastic storages that hold clean food utensils/ equipment in storage area with an accumulation of dust, dirt, food residue. Food employee observed in prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Repeat violation since last inspection. The handwash sink for employees located in the prep/ cook area, is not convenient and easily accessible. Observed a trash can blocking access to the sink and the hand soap dispenser was blocked by bain-marie unit. Corrected on site by moving trash can and adding additional hand soap to sink.

Hampton Inn & Suites Near The Park, 195 Hershey Rd Hummelstown, June 24. Pass. Observed scrambled eggs, potatoes, and sausage, commercially processed food, was only heated to 125.1, 111.8, 112.6 °F before being placed in the steam table and not to 135°F for hot holding as required. Discussed and educated person in charge and corrected on site. Observed the underside of the Juicer self-serve dispenser with accumulatio of sticky residue and in need of cleaning. Observed scrambled eggs, potatoes, and sausage was held at 125.1, 111.8 and 112.6°F, in the hot hold customer self-serve area, rather than 135°F or above as required. Hot hold unit does not have temperature control and is only pre-set from the factory. Unit may be malfunctioning and not keeping foods at the proper temperature. Unit must be replaced or repair to hold foods at the proper hot hold temperature. Discussed and educated person in charge. Food was removed and corrected on site. Observed the top shelf in the upright freezer is broken and in need of repair.

Subway, 67 Outlet Sq Hershey, June 24. Pass. Food employees observed in food assembly area, not wearing proper hair restraints such as beard covers. Observed food (sauces, lettuce) being held at 43°F, in the food assembly area, rather than 41°F or below as required. Corrected on site. Observed multiple dirty knives stored hanging from dirty magnetic knife holder in back prep area. Corrected on site.

Entrance door located next to the check-out area in the front of the store has a gap and does not protect against the entry of insects, rodents, and other animals. Observed missing weather stripping on this door that should be replaced. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed ceiling mounted fluorescent tube lights are that are not shielded or shatter proof over the food prep table in the meat prep area.
