Observed the wall covering in the ware wash area is deteriorating on three of the walls.

Observed deeply scored cutting board on the salad prep unit not resurfaced or discarded as required. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0ppm, and not 50-100 ppm as required. Chlorine sanitizer dispenser was empty. Exposed food preparation observed in kitchen area under dirty ventilation ducts, and subject to potential contamination.
