Burger King #813, 5300 Carlisle Pike Mechanicsburg, June 6. Pass. Observed 4 hot holding units with debris and grease built up in need of cleaning. Observed intake / exhaust air duct near the fryer has a large amount of static dust.

Cold Springs Inn & Brewing Co., 993 Park Place Mechanicsburg, June 6. Fail. Observed numerous foods, including chili being held at internal temperature of 51.4°F, in the Edesa cooling unit, rather than 41°F or below as required. All foods being held in the unit were disposed of. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed baked potatoes stored cooked and wrapped in foil overnight. Corrected on site. Observed static dust accumulation on wall/ceiling above mechanical dishwasher. Foods are served raw or undercooked to the customer’s request; however, a written consumer advisory (on the menu, table tent, or placard) is not provided to the consumer. Observed clean dish racks stored directly on the floor in ware washing area. Loose rubber door gaskets observed on multiple cooling units in the kitchen area and need replacements. Observed the following on food contact surfaces: – Magnetic knife holder storing knives with accumulation of grease/static dust – Shelving storing plates/bowls in cooking area with accumulation of static dust/ food debris.

Dunkin’, 1110 E Simpson St. Mechanicsburg, June 6. Pass. Observed personal belongings stored on an active food prep table in the rear food prep area. Corrected on-site. Food facility is reusing plastic drink cups and lids, which are intended to be a single-service or single-use article. Observed one-time use cups and lids washed and sanitized in the dish area. Operator stated the cups are for employee usage; educated operator on proper use, disposed of cups and lids. Observed wet wiping cloths in the hot food prep area, not being stored in sanitizer solution. Observed clean food storage containers in the dish washing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Corrected on-site. Mops are not being hung to air dry in the slop-sink area. Corrected on-site.

Lin Buffet & Grill, 4880 Carlisle Pike Mechanicsburg, June 6. Fail. Observed 4 severely dented cans stored in the dry storage area. Corrected on site. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed the following in need of cleaning: -Freezer floor has a large amount of debris under racking – the hibachi grill stand has a large amount of grease and build up – Under equipment in the front of the house has food and debris. – inside of the dish machine has extremely large amount of what appears to be lime build up. Observed chicken and fish in the walk-in freezer and vegetables in the walk-in cooler area stored open with no covering. Corrected on site. Observed door located in the back storage area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Observed food ingredient storage containers, throughout the facility, are not labeled with the common name of the food. Food Facility Person in Charge not able to produce records showing that raw or partially cooked ready to eat fish has been frozen by the facility or approved supplier as required, before use or sale. Observed the following: – grey storage container with debris containing knives and small wares that were not clean to sight and touch. – Observed meat grinder with yellow build up on the interior of the unit. – Observed cutting boards with deep scoring and food debris stored clean. Best Western Plus, New Cumberland, 702 Limekiln Road New Cumberland, June 5. Fail. Food Employee did not follow proper handwashing procedure. Soap and warm water not used. Educated at the moment. Water was still not at the correct temperature when hand washing was conducted. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Stated that wash water is only to be 82 degrees instead of the 110 or greater as required. Stated didn’t know the temperature of eggs and sausage heated to serve on buffet and no thermometers were available. A Food Employee was observed washing their hands at the three bay sink rather than the designated handwash sink. Corrected on site. Food Employee observed donning single use gloves without a prior hand wash. Corrected on site. Observed commercially processed food (egg omelet and sausage patties) was only heated to 113.9 and 104.2 °F before being placed in the steam table and not to 135°F for hot holding as required. Educated employee and corrected on site with my equipment. Observed egg omelets and sausage patties being heated in the microwave oven in the kitchen area and placed in a warmer for guests to self-serve. Eggs were only heated to 104.2 and sausage was only heated to 113.9. Discussed with employee and no thermometer to check temperatures of prepared food is available. Educated and correct on site with my equipment. Observed single-service, single-use articles stored in self-serve guest area which were not in the original protective package. Observed forks, knives, plates and bowls not protected or inverted to protect against contamination. Observed food (egg omelet and sausage patties) was held at 113.9 and 104.2 °F, in the self-serve guest area, rather than 135°F or above as required. Educated employee and corrected with my equipment to ensure proper temperature was achieved. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Sponges observed in the kitchen area being used to clean food contact surfaces. Discussed and corrected on site. Observed two drawers with food contact equipment (spoons, can opener, spatula, tongs, coffee lids, etc.) stored very dusty and dirty. Discussed with employee and equipment was disposed. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Observed handle on the Solwave microwave, in the kitchen area, with a build-up of food debris and in need of cleaning. Observed underside [dispener area of the yogurt dispenser in the guest self-serve area with a build-up of food debris. Unwrapped and unprotected single use straws stored in the customer self-serve coffee area.

Kmj Diner Llc, 487 East King Street Shippensburg, June 5. Fail. Food employees observed in Kitchen and Prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Observed Food Employee on his phone, touched raw meat and then do food preparation tasks without changing gloves. Observed employee with gloves on while touching raw meat, change gloves without washing hands. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the walk-in cooler, and held more than 24 hours, is not being marked with the date it was opened. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the Baine Marie, is not being date marked. Observed wet wiping cloths in Kitchen area, not being stored in sanitizer solution. Observed wiping cloths in extremely unclean condition in Kitchen/ Grill area. Observed Frozen Vegetable Mix, Pasta and Fish in the walk-in freezer stored open with no covering. Food dispensing utensil observed stored in the Flour bin and not with handle above the top of the food and the container. Observed Dry Pancake mix stored directly on the floor in storage area, rather than 6 inches off of the floor as required. Observed the reflector plate in the ice machine to have a black/pink slime on it.

This is a repeat violation. Warewashing sink does not have basins large enough to accommodate the largest piece of equipment needing manual warewashing. This is a repeat violation. Observed a black mold-like substance on the interior of the dispensing nozzle of the slushie machine. Discussed and educated with person in charge and was corrected on site. Food employee (dish-washer in the back kitchen area) observed storing clean food equipment while wet, and not allowing time for draining or air-drying. Food employees observed in kitchen area, not wearing proper hair restraints, such as beard covers. Plumbing system not maintained in good repair – observed hand-wash sink in the front kitchen area leaking at the drain pipe and in need of repair. Multiple dry food storage containers (sugar, salt, flour, panko, etc.) with single use no handled containers inside. Discussed with management and corrected on site. Food utensils in front and back of the kitchen area observed stored in a container of water which is not maintained at 135°F. Educated and corrected on site. Observed numerous wet wiping cloths in front and back of the kitchen areas, not being stored in sanitizer solution. Observed one knife stored dirty on a magnetic knife holder at the end of the back kitchen area. Corrected on site. Two large ice scoops observed stored on top of the ice machine, which was not a clean surface. Discussed with management. Scoops were removed and corrected on site. Women’s toilet room is not provided with a covered waste receptacle for sanitary napkins. Due to all three toilet rooms are uni-sex, a covered waste receptacle must be in each room.

Cc’s Kitchen, 710 W. Main St Mechanicsburg, Regular/Complaint, June 3. Fail. Observed tomato sauce and gravy in steam table in cooking area held at 75°F, and 64°F, rather than 135°F or above as required. Observed large accumulation of debris/grease on nonfood contact surfaces of equipment (grill, stove, shelves, floors) in cooking area and need cleaning. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed multiple foods in steam table in cooking area not reheated to 165°F within two hours, for hot holding. The handwash sink in the cooking area was blocked and not accessible at all times for employee use. Corrected on site. Soap was not available at the handwash sink in the prep area. Corrected on site. Observed numerous foods in the Avantco refrigerator in cooking area stored uncovered and unprotected. Plumbing system not maintained in good repair – observed faucet fixture for handwash sink in cooking area loose/broken and needs repairs. Observed the following on food contact surfaces: – Handles for handwash sink cooking area with accumulation of old dried food residue – Dials for Avantco oven burners in cooking area with accumulation of old dried food residue – Interior surfaces of ice machine with accumulation of dirt/pink biofilm – Manual can opener blade with heavy accumulation of dirt/food debris. Corrected on site – Automatic slicer with accumulation of old food debris on blade and food contact surfaces. Corrected on site – Racks in walk in cooler with accumulation of dirt/food debris and need more frequent cleaning – Container storing steak knives in front service area with accumulation of crumbs/food debris. Corrected on site – Lids for bottles of sauce stored in bain-marie unit in cooking area with accumulation of dirt/old food residue and need more frequent cleaning. Corrected on site – Scoop for ice stored directly on top of ice machine, which is not a clean surface. Corrected on site. Potential rodent harborage areas inside the food facility observed in the entire kitchen area due to numerous rodent-like droppings on floors. Dairy Queen #16189, 3615 Market St. Camp Hill, June 2. Fail. Candy ice cream decorations in the two drawers at the ice cream cake assembly area stored open with no covering and non-food contact items placed on top of candies. Numerous containers of frozen toppings in the M3 Turbo Air two door upright freezer unit, stored uncovered. A phone and charger were stored directly on a prep/assembly stainless table on the cook line. Old unused equipment stored in facility, should be removed from food facility. Observed several food equipment items not being stored clean and covered or removed from facility. Discussed with Management. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Plumbing system not maintained in good repair – observed faucet leaking at the three bay sink. Two male food employees observed in food prep/cook/assembly areas, not wearing proper hair restraints, such as nets, hats, or beard covers. Exit door located in the back of the facility area of the food facility has a gap (gasket is deteriorated around perimeter bottom half of door) and does not protect against the entry of insects, rodents, and other animals. Observed the underside and wired guard of the table top mixer with dried food debris and not clean to sight or touch. Interior bottom of Norlakes baine marie cooling unit at the sundae assembly area is in need of cleaning. Numerous food ingredient storage containers, bottles and shakers, in the entire facility, are not labeled with the common name of the food. Observed single-service, single-use articles stored in entire facility area which were not in the original protective package or inverted to protect against contamination. Discussed with management and some, not all were corrected at this visit. Observed the following on food contact surfaces: A brown and black mold-like substance on the ice shute of the self-serve Coca-Cola machine and not clean to site or touch. Underside of soft serve ice cream dispenser with build-up of a black mold-like slime with the possibility to contaminate cones. Corrected on site. Accumulation of food debris in most of the drawered food contact utensil drawers in the front and back of the facility, and in need of cleaning. Vegetable slicer with dried food debris on a shelf under the prep/cook area. A sweatshirt was stored on a bottom shelf at the ice cream cake assembly area. Sweatshirt was coming in contact with not-inverted and not in protective plastic ice cream cake containers. Corrected on site. Two stacks of open ice cream cones and four open stacks of cups stored directly on top of the dirty ice cream dispenser. Discussed with management and items were disposed of. Observed numerous food items (commercially processed and food prepared in the food facility) in all of the cooling units, and being held for more than 24 hours, is not dated marked with the date it was opened or made. Observed medication, chargers, a pocket knife, batteries, etc. stored on a shelf with food across from the ice machine in the back room. Corrected on site. Observed a cell phone stored directly on a prep table near the three bay sink area. Corrected on site Observed numerous employee food items co-mingled with facility food items. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Entire back kitchen area is in need of a thorough cleaning. Shelving with heavy food debris (to the point that food containers are sticking to the shelving), door handles, interiors of cooling equipment, etc. Cloth curtain between front and back of the facility is covered in dry food debris with the possibility to contaminate food and hands. Interior and exterior of Hamilton Beach Microwave is filthy and and in need of a deep clean. Interior shelving of Dukers two door under counter cooling unit with heavy dried milk residue on the shelving and in need of cleaning. Observed wet wiping cloths in front and back areas of the facility, not being stored in sanitizer solution. Some, not all were corrected. Observed hand-wash sink in the back kitchen area filled with two bags of sanitizer, rubber gloves, lids, etc. meaning that no-one is washing their hands. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets or hats. Plumbing system not maintained in good repair – observed drain pipe leaking at the three bay sink area and in need of repair. Observed single-service, single-use articles (two cases of cups stored in kitchen area directly on the floor, and not 6 inches above the floor. Observed single-service, single-use articles stored in kitchen area which were not in the original protective package or inverted to protect against contamination. Observed food ingredient storage containers, shakers, etc., in the front and back of the facility, is not labeled with the common name of the food. Food dispensing utensil in the ice bin in the front of the facility observed stored in the food and not with handle above the top of the food and the container. Manual warewashing three bay sink was not set up at this visit. Observed employee wash a bowl without proper soap, rinse and sanitizer. Educated that sinks needed to be cleaned before setting up and proper procedure should be in place. Observed the following on food contact surfaces: Three large stainless kettles and four smaller stainless kettles with heavy tea staining and not being thoroughly cleaned between use. Plastic container of utensils on a shelf over the tea brewer with crumbs in the container coming in contact with the utensils stored. Two white cutting boards with heavy black mold-like substance and scoring and need replaced.
