Italian Village Pizza, Usa, 2 Lehman St. Lebanon, May 30. Pass. Toilet tissue was not on a dispenser. OTC medical supplies were stored on a shelf with food equipment. Corrected. A few obsolete items and small pieces of equipment were observed.

M&N Deli Meat Market, Inc., 201 Lehman St. Lebanon, May 30. Pass. Toilet tissue is not on the dispenser. A wooden door next to the meat cutting saw is in very poor condition. It can no longer be properly cleaned and is not suitable to be in a food facility.
