Educated and corrected on site. Observed the exterior and interior of the SolWave microwave in the back prep area with dried food debris and the possibility to contaminate gloved hands and food being prepared inside.

Valley Bistro, 4520 Valley Rd. Enola, April 2. Pass. Food debris and residue observed on the can opener blade on the food prep table in the rear food prep area. Corrected on-site. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed food residue build-up on the handles of the refrigerated units in the main food prep area/cooking line. Plumbing system not maintained in good repair – observed a leaking faucet at the utility sink in the food prep area. Ceiling in the main food prep area/cook line area has a hole adjacent to the hood vent system. Hole is repaired with tape which is hanging down from the ceiling above a food prep area. Operator to correct ASAP.

Asian Choice Grocery, 4610 Carlisle Pike Mechanicsburg, April 1. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. PIC was unable to provide documentation of various unidentified meats, possibly goat, and chicken products stored in a backroom freezer as well as in a freezer in an unapproved kitchen. Food Facility has packaged food which is not labeled as required. 10-15 bags of snacks and 3 bags of frozen product Pulled by PIC. Observe the following foods stored open with no covering: -Dried fish observed stored open in an unapproved grocery style bag -Frozen meat products stored in open unapproved garbage style bag -coin nuts in liquid stored on a stool -multiple spices in a storage room on a black table. Food facility does not maintain and implement written procedures and plans as required in the PA Food Code. Observed fried and seasoned pasta cooked on premises packaged with not proper label. Educated PIC on proper licensing and procedures. -Observed boxes of tea toast, potatoes, kidney beans, poha, ginger, ramen and limes stored directly on the floor in the back room area, rather than 6 inches off of the floor as required. -observed food and food storage containers not separated from chemicals a potential for chemical contamination. Observed backstock of food and beverages stored in unclean rooms in the backroom of the facility. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the baine marie, and held more than 24 hours, is not being marked with the date it was opened. Observed in unapproved Kitchen: -There is no approved warewashing system to properly wash, rinse and sanitize equipment -Observed a single bay sink with no handwashing sign, single use towel/air drying device. Observed the following on food contact surfaces: -multiple dirty knives in the back room and in unapproved kitchen -multiple utensils in the unapproved kitchen and in random locations throughout the backroom not clean to sight or touch and/or not stored properly to prevent contamination. – a grinder with a large amount of dried food residue on the floor -unapproved prep table with dried debris – extreme build-up of organic material on a cutting board being stored on a milk crate on the floor -tongs on the floor of back room with a large amount of dried organic material -cutting board on baine marie with organic material and deep scoring. Observed single-service, single-use articles stored in backroom area directly on the floor, and not 6 inches above the floor. The food facility does not maintain Food Employee Certification records as required. Food Facility does not have any employees who hold a Certified Food Manager certificate from a recognized certification program. Observed the following floor concerns: -the main back-room floor is made of concrete that is not smooth -and easily cleanable. -several rooms off the main backroom are made of tile, many are broken and not easily cleanable. Observed 4 exterior doors in need of seals/gaskets to prevent rodent/insect infestation. Observed holes in exterior walls throughout the backrooms all in need of repair to prevent rodent/insect infestation. The handwash sink in the bathroom area does not have soap, single use towels/ air drying device or handwash sign. Observed old/broken/unused equipment in multiple areas of the backrooms needing removed including but not limited to: freezers, construction materials, and debris. Observed large amount of dust/dirt/build up on the floors in all back-rooms. Mops are not being hung to air dry.

Camp Hill Mini Mart Inc, 4501 Trindle Rd. Camp Hill, April 1. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. commercially processed Taquitos and Hotdogs were being held between 94-110 degrees. They were not reheated to 165 degrees Fahrenheit within 2 hours for hot holding. Hotdogs and Taquitos between 94-100 degrees Fahrenheit, in hot hold unit, rather than 135  or above as required. Observed the following S&K prepared sandwiches held beyond the expiration date: 3- small ham expired 3/25, 3- lg. Italian expired 3/29, 2-small Italian expired 3/31, 2-small chicken expired 3/30. 1 Gallon of Swiss whole milk expired 3/31 discarded by PIC. Observed dried food residue on the rollers on the hot holding unit. PIC stated it is not cleaned daily as required. Observed mold like substance on the Slushy machine between the clear plastic and nozzle attachment. Observed heavy coffee stains in the handwash sink indicating uses other than handwashing.

Food employees observed in coffee production area/ kitchen, not wearing proper hair restraints, such as nets, hats, or beard covers. 2 bags of open cheese and a pan of dessert, ready to eat refrigerated products time/temperature controlled for safety food are being held for more than 24 hours not being date marked.

Food employees observed in kitchen and back prep areas, not wearing proper hair restraints, such as nets, hats, or beard covers. Papa John’s Pizza #795, 931 Martket St. Lemoyne, April 1. Pass. Food employee observed in food prep area not wearing proper hair restraints, such as a hairnet or hat. Corrected on-site. Observed cheese, spinach, pepperoni and chicken wings being held at 51 degrees Fahrenheit, in the lower portion of the bain-marie unit in the food prep area, rather than 41 degrees Fahrenheit or below as required. Coached operator on refrigerated equipment settings, adjusted settings and corrected on-site. Refrigerated unit in the food prep area is leaking water and needs repaired. Operator stated work order has been placed, unit will be repaired 4/5/25. Observed a mop not hung to air dry resting on the floor of the food prep area. Corrected on-site.

Freshido, 6455 Carlisle Pike Bldg 3 Mechanicsburg, March 31. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed numerous food ingredient storage containers, located in the rear storage area, is not labeled with the common name of the food. Observed cooked chicken and beef stored within warmer, was held at 110-120 degrees Fahrenheit, rather than 135 degrees Fahrenheit or above as required. Food discarded at the time of inspection. Refrigerated ready to eat (cut vegetables/meat), time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the walk-in cooler and upright refrigerator, is not being date marked. Observed loose/torn rubber door gaskets within numerous refrigerator units (located on cooks’ line) in need of replacement or repaired at the time of inspection. Observed numerous deeply scored cutting boards not resurfaced or discarded as required. Observed mechanical warewashing unit having a large build-up of filth and food residue around the top area and not cleaned before use, and frequently throughout the day. Observed numerous utensils and storage holders soiled with old food/ dirt residue and not clean to the sight and touch. Observed bottom shelving and sliding door jams of upright TRUE refrigerator unit, has an accumulation of liquid splash, dirt and old food residue and in need of cleaning. Observed single-service(bowls/lids), single-use articles stored in rear area directly on the floor, and not 6 inches above the floor. Observed clean food equipment(stainless steel containers/bowls) located in and/or utensils in front and rear areas, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Observed handsink located in rear area, is not producing enough pressure and causing water to trickle when turned to the on position. Observed flooring throughout the entire food facility, is heavily soiled with dirt, dust, trash and old food debris and in need of cleaning at this time.

Harrisburg Academy, 10 Erford Rd. Wormleysburg, March 31. Pass. Walk-in cooler’s floor is separating at the union of sheet metal yielding a harborage site where moisture, crude and micro-organisms may collect. Facility needs to repair and seal off the separation by the start of the next school year in September, 2025.

Luna Italian Cuisine, 100 Legacy Park Dr. Mechanicsburg, Change of Owner, March 31. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed multiple foods being held at 59 degrees Fahrenheit, in the Avantco bain marie unit in the cooking area, rather than 41 degrees Fahrenheit or below as required. Plumbing system not maintained in good repair – observed water leaking from the PVC piping underneath the handwash sink in food prep area. Paper towel dispenser empty at the handwash sink in the food prep area.

Psh Hampden Medical Center, 2200 Good Hope Rd. Enola, March 31. Pass. Observed two pizza paddles stored on top of the pizza oven with the paddle blades resting directly on the top of the unit. Pizza oven observed with a build-up of old food debris and dust and was otherwise not clean to sight and touch. Corrected on-site. Observed a pizza oven in the food prep area, with an accumulation of dust, food residue and debris on non-food contact surfaces.
