(Carlisle) -- Federal investigators are looking into a fatal accident at an Amazon warehouse in the midstate.

Cumberland County Coroner Charley Hall said Amazon employee Jody Rhoads was operating a motorized pallet jack when it crashed into shelving and pinned her.

Hall said an autopsy indicated the 52-year-old Carlisle woman died of multiple traumatic injuries as a result of Sunday's accident.

The incident is under investigation by the Occupational Safety and Health Administration.

Amazon said it is working with OSHA to determine what happened.
